背景:全膝关节置换假体包括固定平台假体和旋转平台假体,其中旋转平台假体的结构更符合人体生物力学特征,但旋转假体是否更优于固定平台假体,仍有争议.目的:探讨Gemini MKⅡ旋转活动平台假体在全膝关节置换中的应用效果.方法:选取我院2013年11月至2015年11月接受全膝关节置换术患者88例,采用随机数字表法分为两组,每组44例.对照组采用固定平台假体,观察组采用Gemini MKⅡ旋转活动平台假体.比较两组患者的手术指征、功能评定、关节活动度、临床疗效、并发症.结果:治疗后,两组患者HSS评分(疼痛、功能、活动度、肌力、屈曲畸形、稳定性、总分)、屈曲活动度相比术前均显著改善增加(P<0.05),伸直活动度降低(P<0.05).观察组患者HSS评分(疼痛、功能、活动度、肌力、屈曲畸形、稳定性、总分)、屈曲活动度、治疗优良率显著高于对照组(P<0.05).而观察组患者伸直活动度、并发症发生率显著低于对照组(P<0.05).观察组患者术后住院时间少于对照组(P<0.05).结论:Gemini MKⅡ旋转活动平台假体应用与全膝关节置换后,可明显改善患者病症,提高关节功能.“,”Background:There are fixed platform prosthesis and rotating platform prosthesis used in total knee arthroplasty (TKA). The structure of rotating platform prosthesis accords with biomechanics. However, whether rotating prosthesis is su-perior to fixed platform prosthesis is still controversial. Objective:To investigate the clinical outcome of Gemini MK Ⅱro-tating platform prosthesis in TKA. Methods:A total of 88 patients undergoing TKA in our hospital from November 2013 to November 2015 were enrolled in this study. They were divided into two groups by random number table method (n=44). The fixed platform prosthesis was used in control group and Gemini MK Ⅱrotating platform prosthesis was applied in ob-servation group. Surgical indications, functional evaluation, range of motion of the knee joint, clinical efficacy and compli-cations were compared between two groups. Results:HSS scores (pain, function, activity, muscle strength, flexion deformi-ty, stability, total score) and range of flexion significantly increased after surgery in both groups (P<0.05), while the range of extension significantly decreased (P<0.05). HSS scores (pain, function, activity, muscle strength, flexion deformity, stabili-ty, total score), range of flexion, excellent treatment rate in observation group were significantly higher than those in control group (P<0.05). The range of extension, incidence of complications and postoperative hospitalization time in observation group were significantly lower than those in control group (P<0.05). Conclusions:Gemini MK Ⅱrotating platform prosthe-sis can obviously ameliorate symptoms and improve the joint function in patients undergoing TKA.
