目的 :探讨转化生长因子 β1(TGF β1)在颅脑损伤患者病程中的变化及作用。方法 :采用ELISA方法 ,对 5 5例急性颅脑损伤患儿 (轻、中、重型 )分别于伤后 1、3、7d( 10例患者另于伤后 2、3周 )检测血清TGF β1含量。结果 :伤后 2 4h内病例组血清TGF β1含量[( 4 6 2 2± 11 64 ) μg·L-1]明显低于对照组 [( 76 2 2± 12 10 ) μg·L-1] ,并有显著性意义 (t =19 74,P <0 0 0 1) ,伤后 3dTGF β1降至最低水平 [( 3 8 62± 10 68) μg·L-1] ,伤后 7d有所回升 [( 5 6 42± 12 0 8) μg·L-1] ,但仍低于对照组水平并有显著性意义 (P <0 0 0 1) ;TGF β1的变化与颅脑损伤的程度有关 ,与对照组比较轻型患者血清TGF β1水平降低不明显 (P >0 0 5 ) ,中、重型患者随病情加重血清TGF β1水平递减 ,重型患者不但血清TGF β1水平低 ,而且持续时间长 ,恢复缓慢。结论 :颅脑损伤患者血清TGF β1降低 ,可能与创伤后脑细胞和血脑屏障受损有关。
Objective: To investigate the change and role of transforming growth factor-β1 (TGF-β1) in the pathogenesis of craniocerebral injury. Methods: Serum levels of TGF-β1 in 55 children with acute craniocerebral injury (mild, moderate, and severe) at 1, 3, and 7 days after injury (the other 10 or 2,3 weeks) . Results: The serum level of TGF-β1 in the cases within 24 hours after injury was significantly lower than that in the control group [(4622 ± 1164) μg · L-1] [(4262 ± 1164) μg · L-1] (T = 1974, P <0.01). The level of 3dTGFβ1 decreased to the lowest level (382 ± 1068μg · L-1) after injury, and it recovered at 7d after injury [ (5 6 42 ± 12 0 8) μg · L -1], but still lower than the control group (P 0 01). The change of TGF β 1 was related to the degree of brain injury and Serum levels of TGF-β1 in the control group were not significantly reduced (P> 0.05). Serum levels of TGF-β1 decreased with the progression of the disease in the moderate and severe patients. Serum levels of TGF-β1 were not only low, but also prolonged and recovered slowly. Conclusion: The decrease of serum TGFβ1 in patients with craniocerebral injury may be related to the damage of brain cells and blood-brain barrier after trauma.
