[目的]通过Meta分析比较前交叉韧带(anterior cruciate ligament,ACL)损伤自体韧带重建残端保留与不保留临床疗效的差异。[方法]检索并收集2000年1月~2016年10月公开发表在Pubmed、Cochrane图书馆、Springer Link、万方数据库、中国知网(CNKI)以及中文科技期刊全文数据库(VIP)关于交叉韧带损伤自体韧带重建残端保留与不保留临床疗效比较的文章,严格按照纳入和排除标准及文献质量评分标准收集数据,运用Review Manager 5.3统计学软件对所提取的文献数据进行meta分析。[结果]共纳入15篇文献。Meta分析显示:前交叉韧带损伤自体韧带重建残端保留术对患者膝关节的客观稳定性KT-1000/2000测量[MD=-0.21,95%CI(-0.25,-0.16),P<0.01]、早期本体感觉恢复[MD=-0.72,95%CI(-1.16,-0.28),P<0.01]、后期本体感觉恢复[MD=-0.45,95%CI(-0.66,-0.25),P<0.01]明显好于非保留组;主观满意度Lysholm评分[MD=0.20,95%CI(-0.13,0.53),P=0.24]、Cyclops病发生率[RR=1.12,95%CI(0.36,3.52),P=0.84]、手术时间[MD=-4.49,95%CI(-38.85,29.88),P=0.80]两种手术差异有统计学意义。[结论]前交叉韧带损伤自体韧带重建残端保留术对患者膝关节的客观稳定性,早、远期本体感觉恢复有明显的促进作用;保留与不保留术对患者的主观满意度、Cyclops病发生率、手术时间没有明显影响。
[Objective] To compare the difference of clinical curative effect between autologous ligament remodeling residual stump and left anterior cruciate ligament (ACL) injury by Meta-analysis. [Methods] The data of cruciate ligament injury (CRF) in the Pubmed, Cochrane Library, Springer Link, Wanfang Database, CNKI and Chinese Science Journals were searched and collected from January 2000 to October 2016. Autologous ligament reconstruction stump retained and did not retain the clinical efficacy of the article, in strict accordance with the inclusion and exclusion criteria and the quality of the document scoring data collection, the use of Review Manager 5.3 statistical software meta-analysis of the extracted literature data. [Results] A total of 15 articles were included. Meta-analysis showed that the objective stability of KT-1000/2000 measured in patients with knee anterior cruciate ligament reconstruction with anterior cruciate ligament injury [MD = -0.21,95% CI (-0.25, -0.16), P <0.01] (MD = -0.72, 95% CI (-1.16, -0.28), P <0.01], postoperative proprioceptive sensation [MD = -0.45,95% CI (-0.66, -0.25), P < 0.01] was significantly better than the non-retention group; subjective satisfaction Lysholm score [MD = 0.20,95% CI (-0.13,0.53), P = 0.24], the incidence of Cyclops disease [RR = 1.12,95% CI ), P = 0.84]. The time of operation [MD = -4.49,95% CI (-38.85,29.88), P = 0.80] showed significant difference between the two operations. [Conclusion] Anterior cruciate ligament injury with autologous ligament remodeling stump retention on the patient’s objective stability of the knee, early and long-term proprioceptive improved significantly; patients with retention and unreserved subjective satisfaction, Cyclops disease The incidence of surgery time did not significantly affect.
