目的:调查与探讨西药房药品发放差错的影响因素。方法:2014年7月到2016年10月,对我院西药房发生的差错进行回顾性调查分析,总结发生差错的类型与原因。结果:西药房共发放药品129830次,其中123次出现发放差错,出错率为0.09%,其中药品差错60次,剂型差错40次,剂量差错23次。在123次药房发放差错中,其中药品摆药错误60次,药品调配错误5次,处方核对错误19次,处方审核错误39次。结论:药房的药品发放准确性对患者的生命安全具有重要的作用,不过当前发放差错也比较常见,我们要尽量减少药品差错的发生,提高西药房的工作质量。
Objective: To investigate and explore the influencing factors of drug dispensing error in western pharmacy. Methods: From July 2014 to October 2016, we retrospectively analyzed the errors that occurred in our western pharmacy to summarize the types and causes of the errors. Results: A total of 129,830 medicines were distributed in western pharmacies, of which 123 were issued errors, with an error rate of 0.09%, including 60 drug errors, 40 dosage errors and 23 dosage errors. In the 123 pharmacies to issue errors, including drug misplacing 60 times, 5 times the wrong drug deployment, prescription check error 19 times, the prescription audit error 39 times. Conclusion: The accuracy of drug delivery in pharmacies plays an important role in patients’ life safety. However, the current issuance errors are also common. We should try our best to minimize the occurrence of drug errors and improve the quality of western pharmacy work.
