目的:探讨围绝经期女性骨密度(BMD)与雌二醇和卵泡刺激素(FSH)水平的关系。方法:选取2011年9月~2012年9月在北华大学附属医院妇产科门诊就诊的392例女性,对其卵泡刺激素(FSH)、雌二醇水平及腰椎、总髋部、股骨近端的BMD进行测量。结果:绝经后女性各骨骼部位的BMD均明显低于绝经前女性,差异有统计学意义(P<0.05);绝经前、后低骨量组女性体重及体重指数(BMI)明显低于正常骨量组女性,FSH水平明显高于正常骨量组女性,差异均有统计学意义(P<0.05);绝经前、后女性腰椎BMD从51岁起开始降低,股骨颈BMD从56岁起开始降低,与之前年龄段女性相比,差异均有统计学意义(P<0.05);绝经前女性雌二醇水平从46岁起明显降低,FSH水平从46岁起明显升高,与之前年龄段女性相比,差异均有统计学意义(P<0.05);绝经后女性雌二醇水平从51岁起明显降低,FSH水平从51岁起明显升高,与之前年龄段女性相比,差异均有统计学意义(P<0.05);雌二醇水平、体重与BMD呈正相关;FSH水平、年龄、绝经时间与BMD呈负相关。结论:雌二醇和FSH水平均与BMD密切相关,绝经后FSH水平与BMD进一步丢失有关,而低雌二醇水平不再是继续影响骨量的主要因素。
Objective: To investigate the relationship between bone mineral density (BMD) and the levels of estradiol and follicle stimulating hormone (FSH) in perimenopausal women. Methods: From September 2011 to September 2012, 392 women attending obstetrics and gynecology clinics of Affiliated Hospital of Beihua University were enrolled. Their FSH, estradiol levels, lumbar spine, total hip and femur End of the BMD measurement. Results: The BMD of each bone of postmenopausal women was significantly lower than that of premenopausal women (P <0.05). The body mass and body mass index (BMI) of premenopausal and postmenopausal women with low bone mass were significantly lower than those of normal bone The FSH levels in female and female group were significantly higher than those in normal bone mass group (P <0.05). BMD of lumbar spine in premenopausal and postmenopausal women decreased from 51 years old and BMD of femoral neck decreased from 56 years old (P <0.05). The level of estradiol in premenopausal women decreased significantly from 46 years old, and the level of FSH increased significantly from 46 years old. Compared with the female of the previous age group (P <0.05). The level of estradiol in postmenopausal women decreased significantly from 51 years old and the FSH level increased significantly from 51 years old, which was significantly different from that in the previous age group (P <0.05). The level of estradiol and body weight were positively correlated with BMD. The level of FSH, age and menopause were negatively correlated with BMD. CONCLUSIONS: The levels of estradiol and FSH are closely related to BMD. The level of FSH in postmenopausal women is related to the further loss of BMD. However, low estradiol level is no longer the main factor for the continued effect on bone mass.
