单侧颈动脉阻断后,可造成阻断侧大脑半球脑血流量(CBF)下降,脑血管自动调节功能障碍,脑皮层组织氧合作用受损以及脑神经细胞的缺血性病理改变。临床上对单侧颈动脉阻断后脑缺血情况的判定有多种方法,但没有一种方法是绝对可靠的,常用方法是同时监测脑电图、颈动脉残存压和CBF。缩短阻断时间、避免低氧和低血压、应用脑保护剂是防治脑缺血的关键。
Blocking the unilateral carotid artery can result in decreased cerebral blood flow (CBF) in the blocked hemisphere, impaired cerebrovascular autoregulation, impaired cerebral cortical oxygenation, and ischemic pathological changes in brain neurons. Clinically, there are a variety of methods for judging cerebral ischemia after unilateral carotid artery occlusion. However, none of them is absolutely reliable. The common method is to monitor EEG, carotid artery residual pressure and CBF simultaneously. Shorten the blocking time, to avoid hypoxemia and hypotension, the application of brain protective agent is the key to prevention and treatment of cerebral ischemia.
