目的 探讨妊娠期凝血功能动态变化对产妇产后异常出血的影响.方法 选择2018年6月-2020年6月在该院分娩的妊娠晚期孕妇300例,按照是否出现产后异常出血分为对照组(出血量＜500 ml,270例)和观察组(出血量≥500 ml,30例).比较两组血栓弹力图(TEG)各参数和常规凝血指标水平变化,并对TEG、常规凝血指标两种检测方法诊断预测产后异常出血的准确率和灵敏度进行比较.结果 观察组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)明显高于对照组(P＜0.05),两组凝血酶时间(TT)、纤维蛋白原(FIB)值比较,差异无统计学意义(P＞0.05).观察组凝血因子反应时间(R)、纤维蛋白原反应时间(K)明显高于对照组(P＜0.05),观察组30 min血凝块幅度减少速率(LY-30)、凝固角(α角)、最大振幅(MA)值明显低于对照组(P＜0.05).TEG参数指标方法预测产后异常出血的准确率、灵敏度均明显高于常规凝血指标(P＜0.05).结论 TEG作为可准确反映妊娠期女性机体凝血功能动态变化的检测指标,能应用于辅助临床观察孕妇凝血功能,可作为临床监测产妇产后异常出血倾向的重要指标,有利于指导临床采取相应预防和治疗措施.
