本文观察了子宫内膜异位症(EM)致不孕患者腹腔镜Nd:YAG激光术治疗前后腹腔液前列腺素(PG)和巨噬细胞(Mφ)变化,以及痛经和受孕情况。结果显示,EM致不孕患者腹腔液PG含量和Mφ数量、吞噬活性及酸性磷酸酶(ACP)均显著高于对照组。激光治疗后腹腔液PG含量、Mφ数量、吞噬活性及ACP均显著降低。同时93%患者痛经消失,75%受孕,但受孕者治疗前腹腔液PG含量高于未受孕者。提示:盆腔内异位的子宫内膜可产生大量PG致腹腔液PG含量增加,并可引起大量Mφ侵入腹腔并高度活化,从而引起痛经和干扰受孕。激光治疗去除了异位病灶,可使腹腔液PG含量和Mφ数量和活性降低,有利于改善症状和促进生育;但治疗后受孕率尚受病程长短和合并的不孕因素的影响。
This article investigates the changes of peritoneal fluid prostaglandin (PG) and macrophage (Mφ) before and after laparoscopic Nd: YAG laser treatment in infertile women with endometriosis (EM) and the conditions of dysmenorrhea and pregnancy. The results showed that the content of PG, Mφ, phagocytic activity and acid phosphatase (ACP) in peritoneal fluid of EM infertility patients were significantly higher than those of the control group. After laser treatment, the peritoneal fluid PG content, Mφ amount, phagocytic activity and ACP were significantly reduced. At the same time, 93% of patients disappear dysmenorrhoea and 75% conceive, but the content of PG in peritoneal fluid of pregnant women before treatment is higher than that of non-pregnant women. Tip: pelvic ectopic endometrium can produce a large number of PG caused by peritoneal fluid PG increased, and can cause a large number of Mφ invasion of the abdominal cavity and highly activated, causing dysmenorrhea and interference with pregnancy. Laser treatment to remove the ectopic lesions, peritoneal fluid PG content and Mφ quantity and activity decreased, help to improve the symptoms and promote fertility; but after treatment, the pregnancy rate is still subject to the length of the course and the combined effect of infertility factors.
