目的:比较荧光定量PCR、抗酸染色和结核抗体检测结核分枝杆菌的灵敏度和特异性,及在临床诊断中的应用价值。方法:对91例肺结核病患者和102例非结核呼吸系统疾病患者的痰标本应用荧光定量PCR法和涂片抗酸染色法检测,支气管灌洗液标本抗酸染色法检测,并分离患者外周血血清检测结核分枝杆菌抗体,对3种检测方法的结果进行分析比较。结果:荧光定量PCR法、支气管灌洗液涂片抗酸染色、痰涂片抗酸染色、结核抗体检测阳性率分别为96.7%、47.3%、33.0%、93.4%,特异性分别为98.0%、97.1%、97.1%、92.2%。结论:荧光定量PCR和结核抗体检测对于涂片抗酸染色阴性的活动性肺结核患者有重要的诊断价值。
OBJECTIVE: To compare the sensitivity and specificity of fluorescent quantitative PCR, acid-fast staining and anti-TB antibody in detection of Mycobacterium tuberculosis and its clinical value. Methods: The sputum samples from 91 patients with pulmonary tuberculosis and 102 patients with non-tuberculosis respiratory diseases were detected by fluorescence quantitative PCR and smear antacid staining. The samples of bronchoalveolar lavage fluid were detected by acid-fast staining and the peripheral blood of patients Serum detection of Mycobacterium tuberculosis antibodies, the results of the three detection methods were analyzed and compared. Results: The positive rates of fluorescent quantitative PCR, bronchial lavage smear antacid staining, sputum smear antacid staining and tuberculosis antibody detection were 96.7%, 47.3%, 33.0% and 93.4%, respectively, and the specificity were 98.0% 97.1%, 97.1%, 92.2%. Conclusion: Fluorescent quantitative PCR and TB antibody test have important diagnostic value for active pulmonary tuberculosis patients with smear negative acid-fast staining.
