目的了解兰州市男男性行为人群(MSM)艾滋病(AIDS)流行状况和影响因素,分析MSM艾滋病流行趋势,为艾滋病疫情估计和预测提供相关信息,为制定艾滋病防治策略和干预措施提供科学依据。方法采用MSM志愿者“滚雪球”的方法,对出入酒吧和洗浴场所的MSM进行问卷调查及血清学检测。结果 2006-2010年连续5年共调查889人。其中未婚者574人,占64.57%;已婚者282人,占31.72%;离异或丧偶者33人,占3.71%。共发现艾滋病病毒(HIV)阳性者53例,阳性率为5.96%(53/889);梅毒阳性者121例,阳性率为13.61%(121/889);丙型肝炎(丙肝)阳性者12人,阳性率1.85%(12/650)。2006-2010年,各年MSM人群的HIV阳性率分别为0.92%(1/109)、2.31%(3/130)、7.00%(14/200)、5.50%(11/200)、9.60%(24/250),经χ2检验,差异有统计学意义(P<0.05),HIV感染有上升趋势;各年梅毒阳性感染率分别为0.92%(1/109)、4.62%(6/130)、23.00%(46/200)、18.50%(37/200)、12.40%(31/250),经χ2检验,差异有统计学意义(P<0.01);近3年丙肝阳性率分别为1.50%(3/200)、1.50%(3/200)、2.40%(6/250),经χ2检验,差异无统计学意义(P>0.05)。结论兰州市MSM人群HIV感染率达到了较高水平,安全套使用率低,多性伴、梅毒感染率高等都是促使HIV快速传播的影响因素。
Objective To understand the prevalence and influencing factors of AIDS in men who have sex with men (MSM) in Lanzhou and to analyze the prevalence of AIDS in MSM so as to provide relevant information for AIDS epidemic estimation and prediction and provide a scientific basis for formulating AIDS prevention strategies and interventions. Methods MSM volunteers “snowball ” approach to access bars and bathing places of the MSM questionnaire and serological tests. Results A total of 889 people were surveyed in five consecutive years from 2006 to 2010. Among them, 574 were unmarried, accounting for 64.57%; 282 were married, accounting for 31.72%; and 33 were divorced or widowed, accounting for 3.71%. A total of 53 HIV positive cases were detected, with a positive rate of 5.96% (53/889). 121 cases were positive for syphilis, with a positive rate of 13.61% (121/889) and 12 positive for hepatitis C (HCV) , The positive rate of 1.85% (12/650). The prevalence of HIV in MSM population was 0.92% (1/109), 2.31% (3/130), 7.00% (14/200), 5.50% (11/200), 9.60% (11/200) (P <0.05). HIV infection was on the rise. The positive rate of syphilis in each year was 0.92% (1/109) and 4.62% (6/130), respectively 23.00% (46/200), 18.50% (37/200) and 12.40% (31/250) respectively. There was significant difference between the two groups (P <0.01) byχ2 test. The positive rates of hepatitis C in the past three years were 1.50% 3/200), 1.50% (3/200) and 2.40% (6/250) respectively. There was no significant difference between the two groups (P> 0.05). Conclusion The prevalence of HIV in Lanzhou MSM population has reached a high level, and the low condom use rate, multiple sex partners and high syphilis infection rate are all the influencing factors for the rapid HIV transmission.
