目的 评价经颅多普勒超声 (TCD)对脑底动脉狭窄或闭塞所致短暂性脑缺血发作 (TIA)的诊断价值。方法 5 0例TIA患者进行常规TCD检查 ,发现 39例为颅底单支或多支动脉狭窄或闭塞 (78% ) ,11例正常 (2 2 % ) ,并对其进行了DSA或MRA证实。通过颅底动脉TCD检测 ,确定狭窄或闭塞血管的形态和血流动力学改变 ,并与血管造影对比分析。结果 血管造影显示单支大脑中动脉狭窄 17支、双侧多支血管狭窄 47支及虹吸部闭塞 1支 ;TCD综合分析后发现单支狭窄 17支、多支血管狭窄 6 4支及闭塞 1支 ,符合率 78.7%。结论 TCD检测对于诊断TIA有重要临床意义 ,尤其对中 重度狭窄或闭塞血管的诊断准确性较高 ,单支节段性狭窄准确性更高。
Objective To evaluate the diagnostic value of transcranial Doppler ultrasound (TCD) in detecting transient ischemic attack (TIA) caused by stenosis or occlusion of cerebral artery. Methods Totally 50 patients with TIA underwent routine TCD examination, and 39 patients were found with stenosis or occlusion of the base of the skull base (78%) and 11 patients (22%) with confirmed or confirmed by DSA or MRA. By TCD detection of skull base artery to determine the morphology of stenosis or occlusion of blood vessels and hemodynamic changes, and angiography contrast analysis. Results Angiography showed that there were 17 single artery stenosis, 47 bilateral stenosis and 1 siphon occlusion. TCD analysis showed that there were 17 single stenosis, 64 stenosis and 1 occlusion , In line with the rate of 78.7%. Conclusion TCD has important clinical significance for the diagnosis of TIA, especially in the diagnosis of moderate to severe stenosis or occlusion of the vessel, and the accuracy of single segmental stenosis is higher.
