患者女,73岁。因咳嗽、吐痰30余年,心悸、气喘10余年,加重3天入院。查体:血压11/9kPa,口唇紫绀,颈静脉怒张,胸呈桶状,两肺有干湿罗音,心率85次/min,律整,心音低钝,P_2亢进,腹软,肝肋下4cm,质韧,两下肢有凹陷性水肿。临床诊断为慢性肺原性心脏病,心衰Ⅲ°。
