目的了解浙江省2010年急性腹泻患者中副溶血弧菌(Vibrio parahaemolyticus,VP)的分布与分子分型特征,为副溶血弧菌的分子流行病学研究提供参考依据。方法对急性腹泻病例的粪便标本进行致病菌分离培养,采用纸片扩散法(Kirby-Bauer)开展副溶血弧菌分离株药敏试验,利用多位点序列分型(MLST)进行副溶血弧菌分子分型研究。结果从1 812例腹泻病患者粪便标本中检出病原菌552株,其中副溶血弧菌150株,分离率为8.3%(150/1 812),居细菌性病原第二位。副溶血弧菌感染主要发生在7—9月份,并以18~45岁青壮年发病为主。副溶血弧菌对头孢吡肟和碳氢酶烯类抗生素(亚胺培南、美洛培南)的敏感性达到100%,但对氨苄西林、头孢唑啉耐药率却分别达到了81.3%和63.4%。50株副溶血弧菌菌株MLST分型结果为ST-3型47株,ST-120型2株,ST-8型1株。结论浙江省副溶血弧菌感染在夏秋季急性腹泻中占有重要地位,ST-3是最主要的流行克隆株。长期持续监测副溶血弧菌,对于防控其在环境和人群中的传播具有重要意义。
Objective To investigate the distribution and molecular typing characteristics of Vibrio parahaemolyticus (VP) in acute diarrhea in Zhejiang Province in 2010, and provide reference for the molecular epidemiology of Vibrio parahaemolyticus. Methods Stool samples of acute diarrhea were isolated and cultured. Kirby-Bauer method was used to detect the susceptibility of Vibrio parahaemolyticus isolates. Multi-locus sequencing (MLST) Molecular Classification of Bacteria. Results A total of 552 pathogenic bacteria were detected in stool samples from 1 812 diarrheal patients, of which 150 strains were Vibrio parahaemolyticus, with a separation rate of 8.3% (150/1 812), ranking the second among bacterial pathogens. Vibrio parahaemolyticus infection occurs mainly in July-September, and mainly in young adults aged 18 to 45 years. The sensitivity of Vibrio parahaemolyticus to cefepime and dihydrocarbidine enantiomers (imipenem and meropenem) was 100%, but for ampicillin, cefazolin resistance rates were 81.3% And 63.4%. The results of MLST typing of 50 strains of Vibrio parahaemolyticus showed that there were 47 strains of ST-3, 2 strains of ST-120 and 1 strain of ST-8. Conclusion Vibrio parahaemolyticus infection plays an important role in acute diarrhea in summer and autumn. ST-3 is the most prevalent epidemic strain. Long-term continuous monitoring of Vibrio parahaemolyticus, for the prevention and control of its spread in the environment and population is of great significance.
