目的探索糖尿病性新生血管性青光眼(NVG)的新的治疗方法以提高疗效。方法对11例(13只眼)糖尿病性NVG采用结膜下注射α-干扰素联合青光眼阀门植入手术以及术后及时的眼底光凝治疗。结果11例(13只眼)糖尿病性NVG眼压控制正常,术后1个月平均眼压为17.3mmHg,角膜恢复透明,8例(9只眼)视力不同程度的提高,3例(4只眼)视力不变,无视力下降,虹膜面新生血管完全消退或仅余很少,角膜透明,前房无积血。11例(13只眼)术后1个月后均行全视网膜光凝。结论该方法不仅可消退虹膜面的新生血管,促使前房积血的吸收,恢复角膜的透明,而且可抑制阀门引流盘周围的纤维增生,有利于术后眼压的控制,恢复了部分视力。
Objective To explore the new treatment of diabetic neovascular glaucoma (NVG) to improve the curative effect. Methods Eleven patients (13 eyes) with diabetic NVG underwent subconjunctival injection of α-interferon combined with glaucoma valve implantation and timely postoperative fundus photocoagulation. Results In 11 cases (13 eyes), the intraocular pressure of diabetic NVG was normal. The intraocular pressure was 17.3mmHg at 1 month after operation. The corneal recovery was transparent. The visual acuity of 8 cases (9 eyes) Eyes) unchanged vision, no decline in vision, iris vascularized neovascularization or only very little remaining, corneal transparency, anterior chamber without hemorrhage. Eleven patients (13 eyes) underwent total retinal photocoagulation 1 month after surgery. Conclusion This method can not only retreat neovascularization of the iris, promote the absorption of the anterior chamber hematum, and restore the transparency of the cornea, but also inhibit the proliferation of fibers around the valve drainage disc, which is good for postoperative IOP control and restore some visual acuity.
