目的探讨宫颈液基细胞学(TCT)联合13种高危型人乳头状瘤病毒(HR-HPV)检测对宫颈疾病的筛查能力。方法对2002年6月至2010年5月于我院门诊进行宫颈疾病筛查患者2800例,进行TCT和13种HR-HPV(以下简称HPV)检测,对TCT检查异常、HPV(+)及阴道镜下怀疑病变的324例患者行组织活检,以病理检查结果为标准。结果筛查者平均年龄38.4岁,11.6%的患者TCT≥不典型鳞状细胞(ASCUS),33.1%的患者HPV感染(+)。在阴道镜下多点活检结果:TCT和HPV均(-)者,无高度病变的发生;TCT(-)和HPV(+)者中,≥CINⅡ级的检出率为11.8%(4/34),TCT异常和HPV(+)同时存在的病例中,≥CINⅠ级的检出率最高32.7%(76/232);仅TCT异常而HPV(-)者25%(12/48),且12例均为CINⅡ级,无原位癌和浸润癌的发生。结论 TCT联合HPV检测是宫颈疾病筛查的有效方法。
Objective To investigate the screening ability of cervical liquid-based cytology (TCT) combined with 13 high-risk human papillomavirus (HR-HPV) tests for cervical diseases. Methods 2800 cases of cervical disease screening in our hospital from June 2002 to May 2010 were tested by TCT and 13 kinds of HR-HPV (hereinafter referred to as HPV). The results of TCT examination, HPV (+) and vagina 324 cases suspected microscopic lesions underwent biopsy, pathological examination results as the standard. Results The average age of the screening people was 38.4 years old, 11.6% of the patients had TCT≥ypical squamous cells (ASCUS), and 33.1% of the patients had HPV infection (+). In colposcopy, multi-point biopsy results showed that there was no high degree of pathological changes in TCT and HPV (-), and the positive rate of ≥CINⅡ in TCT (-) and HPV (+) was 11.8% ), TCT abnormalities and HPV (+) coexistence cases, the highest detection rate of ≥ CIN Ⅰ grade 32.7% (76/232); only TCT abnormalities and HPV (-) were 25% (12/48), and 12 Cases were CIN Ⅱ level, no carcinoma in situ and invasive cancer. Conclusion TCT combined with HPV testing is an effective method for screening cervical diseases.
