目的探讨人肝细胞癌组织中PPAR-γ和PTEN蛋白的表达及其临床意义。方法应用免疫组织化学方法检测78例肝细胞癌及21例正常肝组织中PPAR-γ和PTEN蛋白的表达,分析其与肝细胞癌临床病理特征及预后的关系。结果肝细胞癌组织中PPAR-γ蛋白的阳性表达率为51.3%,显著高于正常肝组织(19.0%,P<0.05),PTEN蛋白的高表达率为42.3%,显著低于正常肝组织(90.5%,P<0.05)。PPAR-γ蛋白的表达与肿瘤直径、门静脉癌栓及TNM分期有关(P<0.05);PTEN蛋白的表达与肿瘤直径、门静脉癌栓、淋巴结转移及TNM分期有关(P<0.05,P<0.01)。PPAR-γ与PTEN蛋白表达无明显相关性(r=-0.100,P=0.384)。PTEN蛋白低表达患者术后生存率低于高表达患者(P=0.000);PPAR-γ蛋白的表达与患者术后生存率无关(P=0.175)。COX模型多因素分析结果显示,TNM分期是影响肝细胞癌预后的独立因素。结论肝细胞癌组织中PPAR-γ和PTEN蛋白的表达水平与肝细胞癌的恶性生物学行为密切相关,PTEN有助于评价患者预后。
Objective To investigate the expression of PPAR-γ and PTEN protein in human hepatocellular carcinoma and its clinical significance. Methods The expressions of PPAR-γ and PTEN in 78 HCC and 21 normal liver tissues were detected by immunohistochemistry. The relationship between them was analyzed with the clinicopathological characteristics and prognosis of hepatocellular carcinoma. Results The positive rate of PPAR-γ protein in hepatocellular carcinoma was 51.3%, significantly higher than that in normal liver tissues (19.0%, P <0.05). The high expression rate of PTEN protein was 42.3%, which was significantly lower than that in normal liver tissues 90.5%, P <0.05). The expression of PPAR-γ was correlated with tumor diameter, tumor thrombus in portal vein and TNM staging (P <0.05). The expression of PTEN protein was correlated with tumor diameter, tumor thrombus in portal vein, lymph node metastasis and TNM staging (P <0.05, P <0.01) . PPAR-γ and PTEN protein expression no significant correlation (r = -0.100, P = 0.384). The postoperative survival rate of patients with low PTEN expression was lower than that of patients with high PTEN expression (P = 0.000). The expression of PPAR-γ protein had no correlation with the postoperative survival rate (P = 0.175). COX model multivariate analysis showed that TNM staging is an independent factor affecting the prognosis of hepatocellular carcinoma. Conclusions The expression of PPAR-γ and PTEN in hepatocellular carcinoma is closely related to the malignant biological behavior of hepatocellular carcinoma. PTEN can help to evaluate the prognosis of patients.
