目的测定脓毒症患者血浆中可溶性细胞间黏附分子-1(ICAM-1)和 E-选择素的水平,及其与多脏器功能不全(MODS)评分的相关性,研究其在预后中的预测价值。方法应用双抗夹心 ELISA 法,测定56例脓毒症患者(其中脓毒症组34例,脓毒症并 MODS 组22例)和20例健康人(对照组)血浆中 ICAM-1和 E-选择素的水平,并对其与 MODS 评分进行相关性分析。结果脓毒症组和脓毒症并 MODS 组血浆可溶性 ICAM-1和 E-选择素的水平均明显高于对照组(P<0.05)。脓毒症并 MODS 组患血浆可溶性 ICAM-1和 E-选择素的水平与 MODS 评分呈显著性正相关(P<0.01)。结论脓毒症患者存在明显的血管内皮功能损伤,血浆中可溶性 ICAM-1和 E-选择组可作为脓毒症患者血管内皮损伤的标志物,与 MODS 评分呈显著性正相关,对脓毒症的预后有一定的预测作用。
Objective To determine the levels of plasma soluble intercellular adhesion molecule-1 (ICAM-1) and E-selectin in patients with sepsis and their relationship with multiple organ dysfunction (MODS) scores and to investigate their prognosis Predicted value. Methods The plasma concentrations of ICAM-1 and E-cadherin in 56 patients with sepsis (34 in sepsis, 22 in sepsis and MODS) and 20 healthy controls (control group) Selectin level, and its correlation with the MODS score analysis. Results The levels of soluble ICAM-1 and E-selectin in sepsis group and MODS group were significantly higher than those in control group (P <0.05). The levels of plasma soluble ICAM-1 and E-selectin in sepsis group and MODS group were significantly and positively correlated with MODS score (P <0.01). Conclusions The sepsis patients have obvious vascular endothelial dysfunction. The soluble ICAM-1 and E-selectin in plasma can be used as markers of vascular endothelial injury in septic patients, which is significantly and positively correlated with MODS score. The prognosis has a certain predictive effect.
