目的探讨在社区建立糖尿病健康管理小组对糖尿病患者遵医行为的影响。方法选择2011年1月-2012年1月治疗的糖尿病患者(type 2 diabetes mellitus,T2DM)280例,建立糖尿病健康管理小组,对其进行全面、全程、系统的社区糖尿病健康管理。12个月后比较参与前后患者糖尿病知识掌握情况、饮食控制、遵医用药、定期检查、定期测血糖、合理运动情况及血糖、血脂水平。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果 280例患者糖尿病知识掌握情况、饮食控制、遵医用药、定期检查、定期测血糖、合理运动情况参与前分别为43.93%、50.71%、66.07%、52.50%、49.64%、40.36%,参与后分别为88.57%、83.93%、90.36%、81.07%、92.86%、88.57%,参与前后比较差异均有统计学意义(均P<0.05)。FPG、2 hPG、HbA1c、TC、TG参与前分别为(10.52±1.87)mmol/L、(13.89±2.06)mmol/L、(11.52±1.92)%、(5.06±1.06)mmol/L、(2.01±0.40)mmol/L,参与后分别为(5.67±0.74)mmol/L、(7.26±1.13)mmol/L、(6.39±1.25)%、(3.28±1.76)mmol/L、(1.50±0.40)mmol/L,参与前后比较差异均有统计学意义(均P<0.05)。结论社区糖尿病患者参与健康管理小组活动可有效提高遵医行为,控制病情减少并发症,提高生活质量。
Objective To explore the impact of setting up diabetes health management team in community to observe the compliance of patients with diabetes mellitus. Methods Two hundred and twenty cases of type 2 diabetes mellitus (T2DM) treated from January 2011 to January 2012 were selected to establish a diabetes health management team to conduct a comprehensive, systematic and systematic management of community diabetes mellitus. After 12 months, the patients’ knowledge of diabetes, diet control, compliance with medication, regular examination, regular blood glucose measurement, reasonable exercise and blood glucose and blood lipid levels were compared. Measurement data using t test, count data using χ2 test, P <0.05 for the difference was statistically significant. Results 280 cases of patients with knowledge of the situation of diabetes, diet control, medication compliance, regular examination, regular blood glucose, exercise participation were 43.93%, 50.71%, 66.07%, 52.50%, 49.64%, 40.36% (88.57%, 83.93%, 90.36%, 81.07%, 92.86%, 88.57% respectively). There were significant differences between before and after the intervention (all P <0.05). (10.52 ± 1.87) mmol / L, (13.89 ± 2.06) mmol / L, (11.52 ± 1.92)% and (5.06 ± 1.06) mmol / L before and after FPG, 2 hPG, HbA1c, ± 0.40 mmol / L, respectively, and were (5.67 ± 0.74) mmol / L, (7.26 ± 1.13) mmol / L, 6.39 ± 1.25%, 3.28 ± 1.76 mmol / L and 1.50 ± 0.40, mmol / L, the difference was statistically significant before and after participation (all P <0.05). Conclusion The participation of community diabetes patients in health management group activities can effectively improve the compliance behavior, control the disease to reduce the complications and improve the quality of life.
