目的分析盐城市盐都区艾滋病自愿咨询检测(VCT)求询者人群的相关情况,以及艾滋病病毒(HIV)和梅毒检测结果,为掌握当地艾滋病流行因素,采取针对性预防控制措施提供科学依据。方法收集盐都区2011-2014年各VCT点求询者的个案信息,以及通过VCT点上报的HIV感染者和病人的个案信息,进行流行病学分析。结果 4年共有4635人接受了咨询和检测,共检出HIV抗体阳性21例(0.45%),梅毒抗体阳性149例(3.21%)。配偶/固定性伴阳性、男男性行为史、非商业非固定异性行为史和商业异性行为史人群的HIV抗体阳性率,分别为8.57%(3/35)、3.52%(9/256)、0.89%(4/448)和0.15%(5/3370)。梅毒抗体阳性求询者的HIV感染率(3.36%,5/149)高于梅毒抗体阴性者(0.36%,16/4486)的感染率(χ2=22.49,P=0.000)。经VCT点检出的HIV抗体阳性者首次CD+4T淋巴细胞计数>200个/μL的占80.95%(17/21)。结论盐都区VCT求询者中,性接触为主要的高危行为,需重点加强预防控制性途径传播艾滋病的措施。同时,VCT有利于早期发现HIV感染者/艾滋病病人,应进一步加强和规范咨询检测服务。
Objective To analyze the related situation of HIV / AIDS counseling and testing (VCT) patients in Yandu District of Yancheng City and the detection results of HIV and syphilis so as to provide a scientific basis for targeted prevention and control measures in order to grasp the local AIDS epidemic factors. Methods The case information of VCT participants who were interviewed in Yandu District during 2011-2014 and the case information of HIV infected persons and patients reported by VCT points were collected for epidemiological analysis. Results A total of 4635 people received counseling and testing in 4 years. Totally 21 HIV positive cases (0.45%) and 149 positive cases (3.21%) were detected. The positive rate of HIV antibody was 8.57% (3/35), 3.52% (9/256) and 0.89 (9/256) respectively in the spouse / male with sex partners, men who had sex with men, history of non-commercial nonspecific heterosexual behavior and business history of heterosexual behavior % (4/448) and 0.15% (5/3370). The prevalence of HIV infection (3.36%, 5/149) in syphilis-positive patients was higher than that in syphilis-negative patients (χ2 = 22.49, P = 0.000). HIV antibody positive individuals who were detected by VCT had 80.95% (17/21) of the first CD + 4T lymphocyte counts> 200 cells / μL. Conclusion Among the VCT interrogators in Yandu District, sexual contact is the major high-risk behavior, and the measures to prevent and control sexual transmission of AIDS should be strengthened. At the same time, VCT is conducive to the early detection of HIV / AIDS patients, counseling and testing services should be further strengthened and standardized.
