目的了解甘肃省藏族地区乙型病毒性肝炎(乙肝)病例报告情况,为进一步规范乙肝报告和制定乙肝防控措施提供依据。方法从中国疾病预防控制系统中的疾病监测信息报告管理系统导出甘肃省甘南藏族自治州和武威市天祝藏族自治县2005—2012年乙肝报告发病资料进行分析。结果 2005—2012年甘肃省藏族地区乙肝平均报告发病率为329.58/10万,其中,甘南藏族自治州平均报告发病率为314.39/10万;天祝藏族自治县平均发病率为361.45/10万。2005—2012年来自综合医院的乙肝报告最多,占80.30%;乙肝实验室诊断病例所占比例最多,为82.46%;且随着年份的增长呈逐年上升趋势(P<0.05)。急性乙肝病例所占比例最小,仅为1.97%;但未分类病例最多,占73.04%。结论甘肃省藏族地区乙肝报告质量近年来明显提高,但是乙肝病例大多无法进行分型,因此,应继续加强对临床医生和传染病报告人员的培训,尤其应提高县市级综合医院乙肝分型的能力,以及时掌握该少数民族地区乙肝发病信息和流行趋势。
Objective To understand the status of hepatitis B virus (HBV) cases in Tibetan areas of Gansu Province and provide the basis for further regulating hepatitis B reporting and developing prevention and control measures of hepatitis B. Methods The epidemiological data of hepatitis B in Gannan Tibetan Autonomous Prefecture of Gansu Province and Tianzhu Tibetan Autonomous County of Wuwei City from 2005 to 2012 were derived from the disease surveillance and information reporting management system in China’s disease control and prevention system. Results The average incidence of hepatitis B in Tibetan areas in Gansu Province was 329.58 / 100 000 between 2005 and 2012, of which the average reported incidence was 314.39 / 100 000 in Gannan Tibetan Autonomous Prefecture; and the average incidence rate was 361.45 / 100 000 in Tianzhu Tibetan Autonomous County. The most reported hepatitis B from general hospitals in 2005-2012 was 80.30%. The most cases of hepatitis B laboratory diagnosis were 82.46%. The year-by-year upward trend was observed (P <0.05). Acute hepatitis B cases accounted for the smallest, only 1.97%; but the largest unclassified cases, accounting for 73.04%. Conclusion The quality of hepatitis B reporting in Tibetan areas of Gansu Province has been significantly improved in recent years, but most cases of hepatitis B can not be typed. Therefore, training of clinicians and communicable disease reporting officers should be strengthened. In particular, the hepatitis B genotyping in general hospitals should be improved Ability, and timely grasp of the Hepatitis B incidence information and trends in the ethnic minority areas.
