目的探讨使用氢氯噻嗪联合氯沙坦治疗高血压合并糖尿病的临床效果,为临床治疗提供参考资料。方法选择2014年1—12月在本院住院的高血压合并糖尿病患者158例随机分为试验组和对照组;在使用胰岛素控制血糖的情况下,试验组使用氯沙坦联合氢氯噻嗪,对照组使用氯沙坦进行血压控制。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果治疗1、2、3、4周后,试验组的收缩压及舒张压[(147.40±19.19)、(138.18±19.81)、(127.83±20.95)、(121.55±21.08)mm Hg(1 mm Hg=0.133 k Pa)与(101.87±15.58)、(92.63±15.86)、(83.16±17.18)、(75.87±17.42)mm Hg]均显著低于对照组[(156.21±19.24)、(150.79±19.53)、(145.75±19.97)、(140.41±20.01)mm Hg与(106.14±14.4)、(100.99±14.43)、(96.14±15.14)、(91.57±14.97)mm Hg],差异均有统计学意义(均P<0.05)。治疗后,试验组的血尿素氮与尿微量白蛋白[(5.67±1.38)mmol/L与(25.47±5.06)mg·g-1·Cr-1]均显著低于对照组[(6.25±1.04)mmol/L与(38.73±5.98)mg·g-1·Cr-1],差异均有统计学意义(均P<0.05)。结论临床上采用氢氯噻嗪联合氯沙坦治疗高血压合并糖尿病患者治疗效果较好,能够有效地控制患者血压,降低治疗过程中药物对其他器官的损害,能够有效改善患者预后,可以考虑在临床加以推广应用。
Objective To investigate the clinical effect of hydrochlorothiazide combined with losartan in the treatment of hypertension complicated with diabetes and provide references for clinical treatment. Methods A total of 158 hypertensive patients with diabetes mellitus admitted to our hospital from January to December 2014 were randomly divided into experimental group and control group. When insulin was used to control blood glucose, losartan plus hydrochlorothiazide was used in the experimental group and control group Losartan for blood pressure control. Measurement data using t test, count data using χ2 test, P <0.05 for the difference was statistically significant. Results After 1, 2, 3 and 4 weeks of treatment, the systolic and diastolic blood pressure in the experimental group [(147.40 ± 19.19), (138.18 ± 19.81), (127.83 ± 20.95), (121.55 ± 21.08) mm Hg = 0.133 k Pa) and (101.87 ± 15.58), (92.63 ± 15.86), (83.16 ± 17.18) and (75.87 ± 17.42) mm Hg were significantly lower than those in the control group [(156.21 ± 19.24) and (150.79 ± 19.53) , (145.75 ± 19.97), (140.41 ± 20.01) mm Hg and (106.14 ± 14.4), (100.99 ± 14.43), (96.14 ± 15.14) and (91.57 ± 14.97) mm Hg, respectively, all with statistical significance P <0.05). After treatment, blood urea nitrogen and urinary albumin [(5.67 ± 1.38) mmol / L and (25.47 ± 5.06) mg · g-1 · Cr-1] in the experimental group were significantly lower than those in the control group [(6.25 ± 1.04 ) mmol / L and (38.73 ± 5.98) mg · g-1 · Cr-1, respectively (all P <0.05). Conclusions The clinical use of hydrochlorothiazide combined with losartan in the treatment of patients with hypertension and diabetes is better, which can effectively control the blood pressure of patients and reduce the damage to other organs in the course of treatment, which can effectively improve the prognosis of patients and can be considered for clinical promotion application.
