目的探讨非酒精性脂肪性肝病(NAFLD)患者血清溶血卵磷脂(LPC)水平变化以及其与非酒精性脂肪性肝病的关系。方法选取符合纳入标准的非酒精性脂肪性肝病和正常对照人群各33例。收集并分析两组人群人口学特征以及临床资料,萃取分析血清所含脂质,比较组间血清溶血卵磷脂的相对含量,并对溶血卵磷脂进行多因素非条件Logistic回归分析。结果 NAFLD组LPC 18∶2、LPC 20∶5相对含量均低于正常对照组[(23.99±4.86)μmol/L vs(28.50±6.91)μmol/L,(3.88±0.89)μmol/L vs(4.68±1.33)μmol/L],差异有统计学意义(P<0.01)。多因素Logistic回归分析发现,校正年龄、性别混杂因素,并纳入腰臀比、谷丙转氨酶、甘油三酯以及空腹血糖危险因素后,LPC 18∶2、LPC 20∶5的OR值分别为0.86、0.47,表明血清溶血卵磷脂与NAFLD呈负相关。结论血清溶血卵磷脂与NAFLD呈负相关,LPC可能成为有意义的生物标志物,用于诊断并预测NAFLD的发生。
Objective To investigate the changes of serum lysophosphatidylcholine (LPC) in patients with nonalcoholic fatty liver disease (NAFLD) and its relationship with non-alcoholic fatty liver disease. Methods A total of 33 nonalcoholic fatty liver disease patients and 33 healthy controls were enrolled in this study. The demographic characteristics and clinical data of two groups were collected and analyzed. The lipids contained in the serum were extracted and analyzed. The relative contents of serum lysolecithin in the two groups were compared. Logistic regression analysis was performed on the lysolecithin. Results The relative contents of LPC 18:2 and LPC 20:5 in NAFLD group were significantly lower than those in normal control group [(23.99 ± 4.86) μmol / L vs (28.50 ± 6.91) μmol / L, (3.88 ± 0.89) μmol / L vs ± 1.33) μmol / L], the difference was statistically significant (P <0.01). Multivariate Logistic regression analysis showed that the odds ratios of LPC 18:2 and LPC 20:5 were 0.86 after adjusting for age and gender confounders and included the ratio of WHR, ALT, triglycerides and fasting blood glucose, 0.47, indicating that serum lysolecithin and NAFLD was negatively correlated. Conclusion Serum lysophosphatidylcholine is negatively correlated with NAFLD, and LPC may be a useful biomarker for the diagnosis and prediction of NAFLD.
