[目的]探讨胃肠道间质瘤 (GIST)的免疫组织化学特点及组织分型。[方法]采用微波修复抗原及免疫组织化学SP法检测34例胃肠道间质瘤患者CD34、CD117、vimentin、desmin、SMA、MSA、S_100蛋白、NSE、NF的表达 ,并结合随访资料 ,对其生物学行为进行分析。[结果]34例GIST中良性11例、恶性11例及交界性12例。免疫组化染色证实 :CD117和CD34多为弥漫强阳性 ,阳性率分别为100% (34例 )和82.4% (28例 ) ,vimentin、desmin、SMA、MSA、S_100蛋白、NSE及NF呈片状或局灶阳性 ,阳性率分别为52.9% (18例 )、26.5% (9例 )、29.4% (10例 )、20.6% (7例 )、32.4% (11例 )、20.6% (7例 )、38.2% (13例 )。随访11例恶性组中有3例复发 ,5例死亡 ,其平均核分裂数13个/10HPF。[结论]GIST为起源于胃肠道肌层的最常见间叶性肿瘤 ,免疫组织化学特征为CD117、CD34阳性 ,免疫组化证实仅有部分肿瘤具有不完全的平滑肌、神经或双向分化特征。
[Objective] To investigate the immunohistochemical features and tissue typing of gastrointestinal stromal tumors (GIST). [Methods] The expression of CD34, CD117, vimentin, desmin, SMA, MSA, S_100, NSE and NF in 34 cases of gastrointestinal stromal tumors were detected by microwave repair antigen and immunohistochemical SP method. Its biological behavior is analyzed. [Results] Among the 34 patients with GIST, 11 were benign, 11 were malignant and 12 were borderline. Immunohistochemical staining showed that CD117 and CD34 were diffusely and strongly positive. The positive rates were 100% (34 cases) and 82.4% (28 cases) respectively. The expressions of vimentin, desmin, SMA, MSA, The positive rates were 52.9% (18 cases), 26.5% (9 cases), 29.4% (10 cases), 20.6% (7 cases), 32.4% (11 cases) and 20.6% , 38.2% (13 cases). In 11 cases of malignant group, 3 cases were followed up and 5 cases died. The average number of mitosis was 13/10 HPF. [Conclusion] GIST is the most common mesenchymal tumor originated from the gastrointestinal tract. Immunohistochemical features of CD117 and CD34 are positive. Immunohistochemistry confirmed that only some of the tumors have incomplete smooth muscle, nerve or bidirectional differentiation.
