目的:研究中国人细胞色素P4 502D6(CYP2D6)A、B及Ch进行基因突变分析与帕罗西汀临床效应之间的关系。方法:采用PCR-RFLP法对85例抑郁症患者进行基因型分析,并分组用 HAMD和 TESS评定患者治疗前,治疗 4周和 8周后的抑郁状态及药物不良反应,比较各基因型组患者采用帕罗西汀治疗的疗效和药物不良反应。结果:(1)CYP2D6等位基因频率分别为:Wt(49.5%),Ch(50.0%),B(0.5%)。未检出A等位基因。(2)不同基因型组患者治疗后HAMD评分显著降低;不同基因型患者之间治疗前 HAMD评分、治疗4周和 8周后HAMD评分及减分率,以及TESS评分之间的差异均无显著性意义;患者治疗前HAMD评分、治疗后HAMD评分及减分率、TESS评分等均与年龄、性别、病程、CYP2D6基因型、突变等位基因数目之间的相关性无显著性意义。结论:本研究未发现抑郁症患者CYPZD6基因多态性与帕罗西汀临床效应之间存在相关性。
Objective: To study the relationship between the gene mutation analysis of Chinese cytochrome P450D6 (CYP2D6) A, B and Ch and the clinical effect of paroxetine. Methods: Eighty-five patients with depression were genotyped by PCR-RFLP. HAMD and TESS were used to assess the depression status and adverse reactions of the patients before treatment, 4 weeks and 8 weeks after treatment. Paroxetine treatment with the efficacy and adverse drug reactions. Results: (1) The frequencies of CYP2D6 alleles were: Wt (49.5%), Ch (50.0%), B (0.5%). No A allele was detected. (2) The HAMD scores of patients with different genotypes were significantly decreased after treatment. There was no significant difference in HAMD score, HAMD score and the rate of reduction after 4 and 8 weeks of treatment between different genotypes and TESS scores The significance of HAMD score, HAMD score, reduction rate and TESS score before treatment, age, sex, duration of disease, CYP2D6 genotype and the number of alleles were not significant. Conclusions: This study found no association between CYPZD6 polymorphism and the clinical effect of paroxetine in patients with depression.
