目的探讨慢性乙肝肝硬化患者Child-Pugh分级与肝脏剪切波弹性测量值的相关性。方法收集住院的乙肝肝硬化患者108例,其中Child-Pugh A级48例,Child-Pugh B级32例,Child-Pugh C级28例。所有患者均测量肝脏组织的杨氏模量值,比较不同Child-Pugh分级患者肝组织的杨氏模量值差异,并分析各项杨氏模量值参数与Child-Pugh评分的相关性。结果随着Child-Pugh分级的增加,肝组织杨氏模量值的各项参数均逐渐升高,不同级别患者之间的各项杨氏模量值参数差异均有统计学意义。Child-Pugh评分与肝组织杨氏模量值的各项参数均呈正相关,相关系数分别为平均值r=0.597、最小值r=0.603、最大值r=0.583。结论乙肝肝硬化患者剪切波弹性测量值与Child-Pugh分级有一定的相关性,能在一定程度上反映肝功能的储备情况。
Objective To investigate the correlation between Child-Pugh grading and the shear wave elasticity of liver in patients with chronic hepatitis B cirrhosis. Methods A total of 108 patients with hepatitis B cirrhosis were enrolled. Among them, 48 were Child-Pugh class A, 32 were Child-Pugh class B and 28 were Child-Pugh class C. All patients measured the Young’s modulus of liver tissue and compared the Young’s modulus difference of different Child-Pugh grading patients, and analyzed the correlation between Young’s modulus value and Child-Pugh score. Results With the increase of Child-Pugh classification, the parameters of Young’s modulus of liver tissue gradually increased, and the differences of Young’s modulus between different grades of patients were statistically significant. Child-Pugh score and liver tissue Young’s modulus values of the various parameters were positively correlated, the correlation coefficients were the average r = 0.597, the minimum r = 0.603, the maximum r = 0.583. Conclusion There is a certain correlation between shear wave elasticity measurement and Child-Pugh classification in patients with hepatitis B cirrhosis, which can reflect the reserve of liver function to a certain extent.
