目的:从食管运动角度探讨半夏泻心汤防治反流性食管炎的机制。方法:60只大鼠随机分为伪手术组、模型组、阳性对照组(西沙必利)和半夏泻心汤组(BX)。采用胃-十二指肠混合反流模型;应用多导生理信号记录仪观察离体食管肌条收缩情况;采用RT-PCR的方法观察食管平滑肌收缩调控蛋白基因表达情况;激光共聚焦的方法观察细胞内游离钙的变化。结果:与模型组相比,半夏泻心汤组大鼠食管离体肌条收缩幅度增强;调宁蛋白和钙调蛋白结合蛋白基因表达量降低;细胞内游离钙的浓度增加。结论:半夏泻心汤防治反流性食管炎的机制可能是通过下调调宁蛋白和钙调蛋白结合蛋白基因表达,增加细胞内游离钙的浓度进而增加食管平滑肌收缩能力实现的。
Objective: To explore the mechanism of Banxia Xiexin Decoction in preventing and treating reflux esophagitis from the perspective of esophageal movement. METHODS: Sixty rats were randomly divided into sham operation group, model group, positive control group (cisapride) and Banxia Xiexin Decoction group (BX). The gastric-duodenal mixed reflux model was used; the contraction of isolated esophageal muscle strips was observed with a multi-channel physiological signal recorder; the expression of contractile regulatory protein genes in esophageal smooth muscle was observed by RT-PCR; and the confocal laser scanning was used Changes in intracellular free calcium. Results: Compared with the model group, the contraction amplitude of the isolated esophageal muscle strips in the Banxia Xiexin decoction group was increased, the expression of the calponin and calmodulin-binding protein genes was decreased, and the intracellular free calcium concentration was increased. Conclusion: The mechanism of Banxia Xiexin Decoction in preventing and treating reflux esophagitis may be achieved by down-regulating the expression of calponin and calmodulin-binding protein genes, increasing the intracellular free calcium concentration, and increasing the contractility of esophageal smooth muscle.
