目的研究胰岛素泵治疗对2型糖尿病合并妊娠产妇血糖控制及新生儿的影响。方法选取陕西省人民医院2014年8月-2016年5月收治的2型糖尿病合并妊娠产妇100例,采用随机数字表法将100例2型糖尿病合并妊娠产妇分为观察组(50例)和对照组(50例)。对照组采取皮下胰岛素注射治疗,观察组采用胰岛素泵治疗。观察对比两组患者的血糖控制情况、胰岛素分泌指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)、妊娠并发症以及新生儿并发症。结果观察组的空腹血糖、餐后2 h血糖以及糖化血红蛋白均优于对照组(P<0.05);观察组的HOMA-β与HOMA-IR均优于对照组(P<0.05);观察组产妇的并发症发生率为36.00%,与对照组(64.00%)比较,差异有统计学意义(P<0.05);观察组产妇分娩的新生儿并发症发生率为12.00%,与对照组(30.00%)比较,差异有统计学意义(P<0.05)。结论胰岛素泵能够有效地控制2型糖尿病合并妊娠产妇的血糖水平,减少了多次皮下注射的痛苦,产妇以及新生儿并发症较少,值得进一步推广。
Objective To study the effects of insulin pump on blood glucose control and newborn infants with type 2 diabetes mellitus complicated with pregnancy. Methods 100 cases of type 2 diabetes mellitus complicated with pregnancy admitted to Shaanxi Provincial People’s Hospital from August 2014 to May 2016 were enrolled. One hundred cases of type 2 diabetes mellitus with pregnancy were divided into observation group (50 cases) and control group by random number table Group (50 cases). The control group was injected subcutaneously with insulin, and the observation group was treated with insulin pump. The blood glucose control, insulin secretion index (HOMA-β), insulin resistance index (HOMA-IR), pregnancy complications and neonatal complications were observed and compared between the two groups. Results The fasting blood glucose, postprandial 2-hour postprandial blood glucose and glycosylated hemoglobin in the observation group were better than those in the control group (P <0.05). HOMA-β and HOMA-IR in the observation group were better than those in the control group (P <0.05) (36.00%), the difference was statistically significant (P <0.05) compared with the control group (64.00%). The incidence of neonatal complications in the observation group was 12.00%, which was significantly higher than that in the control group (30.00% ), The difference was statistically significant (P <0.05). Conclusion Insulin pump can effectively control the blood sugar level of type 2 diabetes mellitus complicated with maternal pregnancy, reduce the pain of multiple subcutaneous injections, and have fewer maternal and newborn complications. It is worth further promotion.
