吻合口瘘是直肠癌前切除术后常见并发症,我院2010年行直肠癌前切除术35例,发生吻合口瘘3例,发生率8.6%。文献[1-3]报道引起吻合口瘘的高危因素有:年龄>60岁、男性多于女性、术前合并症(如糖尿病、贫血、营养不良)、肠道准备不充分、吻合口张力大、吻合器使用不熟练、夹芯、术后感染积液等。2011年1月至12月,我们针对常见高危因素,采取围手术期综合处理预防吻合口瘘发生,取得满意疗效,现报告如下。
Anastomotic fistula is a common complication after resection of rectal cancer. In our hospital, 35 cases underwent anterior resection of colorectal cancer in 2010, and 3 cases occurred anastomotic fistula, the incidence rate was 8.6%. Literature [1-3] reported the risk factors for anastomotic fistula were: age> 60 years, more men than women, preoperative complications (such as diabetes, anemia, malnutrition), inadequate intestinal preparation, anastomotic tension , Unskilled stapler, sandwich, postoperative infection fluid. From January 2011 to December 2011, we aimed at the common risk factors and took perioperative comprehensive treatment to prevent the occurrence of anastomotic leakage and achieved satisfactory results.
