应用术中门脉插管测定40例肝硬化病人的FPP,并于术前及术中观测肝脏体积大小;5例病人术前彩超检测门脉血流动力学的改变。结果发现,FPP增高与肝脏体积缩小相关,而与肝功能Child分级不相关;门脉血流量与FPP似有正相关趋势。可见,Child分级敏感性尚不够,需结合肝脏体积以评价肝脏储备能力;而门脉血流增加,对门脉高压的维持有重要作用。
Intraoperative portal vein catheterization was used to measure FPP in 40 patients with liver cirrhosis. Liver volume was observed preoperatively and intraoperatively. Five patients underwent preoperative color Doppler flow imaging to detect portal hemodynamic changes. The results showed that the increase of FPP was correlated with the reduction of liver volume, but not with the classification of liver function. There was a positive correlation between portal blood flow and FPP. Visible, Child grading sensitivity is not enough, the volume of the liver needs to be combined to evaluate the capacity of the liver reserve; and portal blood flow increased, the maintenance of portal hypertension has an important role.
