目的探讨胎盘促肾上腺皮质激素释放激素(corticotropinreleasinghormoneCRH)mRNA水平及脐血硫化去氢表雄酮(DHEAs)含量与早产的相关性。方法选取2002年1月至2003年6月在镇江第四人民医院分娩的初产妇共33例。各组胎盘组织的CRHmRNA采用RTPCR方法测定。脐血DHEAs含量采用ELISA方法检测。结果自发性早产组、医源性早产组胎盘CRHmRNA呈高度表达且显著高于正常产组,差异有显著性意义(P<005);医源性早产组脐血DHEAs含量显著低于正常产组及自发性早产组,差异有显著性意义(P<005);医源性早产组虽CRHmRNA也呈高度表达但其脐血DHEAs含量却显著降低。结论自发性早产的分娩动因可能与胎盘CRHmRNA异常高度表达有关,早产的病理生理机制与CRH在早产时的分泌增加有关;脐血DHEAs含量高低在早产中的意义尚不明确。
Objective To investigate the correlation between placental corticotropinreleasinghormone (CRH) mRNA levels and the content of DHEAs in preterm labor. Methods From January 2002 to June 2003 in Zhenjiang Fourth People’s Hospital delivered a total of 33 cases of primipara. CRH mRNA in placenta of each group was determined by RTPCR method. Umbilical blood DHEAs content was detected by ELISA. Results The CRH mRNA in placenta of spontaneous preterm labor group and iatrogenic preterm labor group were highly expressed and significantly higher than those in normal delivery group (P <0.05). The content of DHEAs in umbilical cord blood in iatrogenic preterm labor group was significantly lower than that in normal birth And spontaneous preterm labor group, the difference was significant (P <005). Although the expression of CRHmRNA in iatrogenic preterm labor group was also high, the content of DHEAs in umbilical cord blood was significantly decreased. Conclusion The motivation of spontaneous preterm labor may be related to the abnormal high expression of CRH mRNA in placenta. The pathophysiological mechanism of preterm labor is related to the increased secretion of CRH during premature delivery. The significance of the content of DHEAs in preterm labor is not yet clear.
