【目的】 探索建立更加适合评价新生儿体格发育的新身体指数。 【方法】 通过 13 4 1例足月适于胎龄 (AGA)儿的身长、体重、头围用计算机建立新的身体指数—张路指数 :体重 (g) +2 5 0头围 (cm)×身长 (cm)× 2 (简称ZLI) ,并通过 14 4例小于胎龄 (SGA)儿、167例大于胎龄 (LGA)儿及 75例足月AGA儿的原始数据对新指数及其它 10项身体指数与贯用的胎龄—体重法进行反馈性验证比较。 【结果】 ZLI随胎龄的增加而增大 ,其中胎龄 3 1~ 3 3周和 3 4~ 3 5周ZLI各组间差异有显著意义 (P <0 .0 5 ) ;ZLI随生后日龄的增大而增加 ,各时段间差异有显著意义 (P <0 .0 1) ;ZLI和其它 10项身体指数与胎龄—体重法比较 ,其灵敏度、特异度和总符合率分别为 70 %、75 %和 73 % ,漏诊率和误诊率分别为 3 0 %和 2 5 %。与其它 10项指数比较 ,正常值波动范围小 ,为 1.0 0± 0 .0 6( x±s) ,均值为 1.0 0容易记忆 ,包含的参量多 (含有新生儿体格发育最重要的三项指标 ) ,更符合综合评价的要求。 【结论】 新建立的ZLI是一个综合评价新生儿宫内及生后 0~ 2月小儿体格发育的好指数。
【Objective】 To establish a new body index that is more suitable for evaluating the physical development of neonates. 【Method】 The body length, body weight and head circumference of 13 4 full-term suitable gestational age (AGA) children were used to establish a new body index - Zhang Lu index: body weight (g) +250 head circumference (cm) × height (cm) × 2 (referred to as ZLI), and through 14 4 cases of small gestational age (SGA), 167 cases of LGA and 75 cases of full-term AGA children’s original data on the new index and other 10 Item body index and the use of gestational age - weight method to verify the feedback comparison. [Results] The ZLI increased with the increase of gestational age, among which there was significant difference between ZLI groups at gestational weeks of 31-31 and 34-55 (P <0.05) (P <0.01). The sensitivity, specificity and overall coincidence rate of ZLI and other 10 body indexes with gestational age-body weight were 70%, 75% and 73% respectively. The rate of misdiagnosis and misdiagnosis were 30% and 25% respectively. Compared with the other 10 indexes, the fluctuation range of the normal value was small (1.0 ± 0.60 (x ± s), with a mean of 1.0 0), which was easy to memorize and included many parameters (including the three most important indicators of physical development in neonates ), More in line with the requirements of comprehensive evaluation. 【Conclusion】 The newly established ZLI is a good index for evaluating the physical development of infants in neonates during the first trimester and the second trimester after birth.
