目的:考察与3种证型2型糖尿病患者的糖尿病相关指标情况,阐明不同证型2型糖尿病的特征。方法:检测3种证型2型糖尿病16项相关指标,并用T检验分析结果数据。结果:痰(湿)热互结型组的体质指数、空腹血糖、糖化血红蛋白、空腹胰岛素、胰岛素敏感指数、胰岛素抵抗指数、胰岛素分泌指数、甘油三脂、高密度脂蛋白、尿蛋白排泄率与对照组具有统计学意义的差异(P<0.05);热盛伤津组和气阴两虚组糖化血红蛋白、空腹血糖和而胰岛素分泌指数与对照组具有统计学意义的差异(P<0.05)。痰(湿)热互结型组体质指数、空腹胰岛素、胰岛素抵抗指数和肌酐高于热盛伤津及气阴两虚组;而胰岛素敏感指数和高密度脂蛋白低于这2组,差异有统计学意义(P<0.05);另外痰(湿)热互结组的甘油三酯和尿蛋白排泄率高于气阴两虚组,总胆固醇低于热盛伤津组,差异有统计学意义(P<0.05)。热盛伤津组和气阴两虚组各指标差别均无统计学意义。结论:早中期2型糖尿病中医辨证分型与患者血糖无明显相关。痰(湿)热互结型在脂代谢和免疫功能异常方面更甚于其他2型,提示其更易于向并发症期转化,治疗难度更大,预后也更差。
OBJECTIVE: To investigate the diabetes-related indicators in patients with type 2 diabetes mellitus and 3 types of syndromes and to elucidate the characteristics of type 2 diabetes mellitus with different syndromes. Methods: Sixteen indicators related to type 2 diabetes mellitus were detected, and the results were analyzed by T test. Results: Body mass index, fasting blood glucose, glycosylated hemoglobin, fasting insulin, insulin sensitivity index, insulin resistance index, insulin secretion index, triglyceride, high density lipoprotein, urinary protein excretion rate in sputum (wet) There was a significant difference between the control group (P <0.05) and the difference of HbA1c, fasting blood glucose and insulin secretion index between the heat Shengjinjin group and Qi-Yin-Yin deficiency group and the control group (P <0.05). The body mass index, fasting insulin, insulin resistance index and creatinine in sputum (wet) heat-interactive type group were higher than those in heat-shock yangjin and qi-yin deficiency group, but insulin sensitivity index and high density lipoprotein were lower than the two groups (P <0.05). In addition, the excretion rate of triglyceride and urinary protein in phlegm (wet) hyperthermia group was higher than those in Qi-Yin-Yin group and total cholesterol was lower than that in Sheng-Sheng-Jin Capsule group, the difference was statistically significant (P <0.05). Heat Sheng Jin group and qi and yin deficiency group indicators were no significant difference. Conclusion: There is no significant correlation between TCM syndrome differentiation and type 2 diabetes mellitus in patients with early-to-middle stage. Phlegm (wet) heat-binding type in the lipid metabolism and immune dysfunction more than other types of 2, suggesting that it is more likely to transition to complications, the treatment more difficult, the prognosis is worse.
