目的 观察不同碘饮食水平对垂体甲状腺功能及外周血甲状腺抗体的影响。方法 选择江苏省徐州高碘病区17~18岁学生,分为高碘甲肿组(HIG)及高碘非甲肿组(HI),选择天津18~19岁学生为适碘对照组(NI)。用放免法测定血清TT3、TT4、FT3、FT4、TGAb、TPOAb,IRMA法测定sTSH浓度,酶免法测定TSI及TRAb。结果 HIG及 HI组血清中sTSH明显高于 NI组;HIG及 HI组 FT3不同程度降低,尤其以 HI组为著;HIG及 HI组 TT3水平代偿性升高;组间 TT4及 FT4差异无显著意义。HIG与 HI组比较,所测定激素指标差异均无显著意义。血清甲状腺抗体结果显示,HIG组 TGAb及 TPOAb较 HI组明显升高,较NI组仅呈升高趋势,差异无显著意义;HIG及HI组TRAb明显高于NI组;HI组TGAb明显低于NI组;HI组的TSI及TPOAb与NI组差异无显著意义。结论 长期摄入过量碘诱发人群亚临床甲状腺功能低下,并使碘致性甲状腺肿患者甲状腺自身免疫反应增强。
Objective To observe the effects of different dietary iodine levels on thyroid function and thyroid antibodies in peripheral blood. Methods A total of 17-18-year-old students with high iodine deficiency in Xuzhou of Jiangsu Province were divided into two groups: high iodine and hypothyroidism group (HIG) and high iodine non-hypothyroidism group (HI) ). Serum levels of TT3, TT4, FT3, FT4, TGAb, TPOAb and IRMA were determined by radioimmunoassay. TSI and TRAb were determined by enzyme immunoassay. Results The serum levels of sTSH in HIG and HI groups were significantly higher than those in NI group. The levels of FT3 in HIG and HI groups were decreased to some extent, especially in HI group. The levels of TT3 in HIG and HI groups were compensatory increased, but there was no significant difference in TT4 and FT4 between groups significance. HIG and HI group, the determination of hormone differences were not significant. Serum thyroid antibody results showed that TGAb and TPOAb in HIG group were significantly higher than those in HI group, but higher than NI group, the difference was not significant; TRAb in HIG group and HI group was significantly higher than that in NI group; TGAb in HI group was significantly lower than that in NI group Group; there was no significant difference between TSI and TPOAb in HI group and NI group. Conclusion Long-term intake of excess iodine induces subclinical hypothyroidism and enhances the thyroid autoimmune response in patients with iodine-induced goiter.
