[目的]探讨DNA聚合酶β表达对食管癌放疗敏感性的影响及预后评估。[方法]收集85例食管癌患者,根据DNA聚合酶β(DNA polβ)的表达水平随机分为高表达常规分割组、高表达超分割组、低表达常规分割组和低表达超分割四组。常规分割:2Gy/f,总量56~64Gy/28~32次,超分割:1.3 Gy/f,2f/d,总量56~62Gy/43~48次。治疗中及治疗后随访,观察急性反应、晚期反应以及近期疗效、局控率及1、3年生存率。[结果]四组之间放疗晚期反应肺纤维化及食管狭窄差别无统计学意义(P=0.887、0.711);近期疗效差异无统计学意义(P=0.862);急性反应放射性气管炎、放射性食管炎差异有意义(P=0.049、0.031),超分割两组显著高于常规分割组;1、3年局部控制率及生存率两组差异明显(P=0.028、0.036、0.040、0.014)。[结论]超分割放疗加重急性反应,DNA polβ表达不同对放疗敏感性有影响,低表达组超分割放疗预后相对较好。
[Objective] To investigate the effect of DNA polymerase β expression on radiosensitivity of esophageal cancer and its prognostic value. [Method] Eighty - five patients with esophageal cancer were collected and randomly divided into four groups according to the expression level of DNA polymerase β: high expression normal group, high expression group, low expression group and low expression group. Conventional segmentation: 2Gy / f, total 56 ~ 64Gy / 28 ~ 32 times, hyperfractionation: 1.3 Gy / f, 2f / d, total 56 ~ 62Gy / 43 ~ 48 times. After treatment and follow-up after treatment, observe the acute response, late response and short-term efficacy, control rate and 1, 3-year survival rate. [Results] There was no significant difference in pulmonary fibrosis and esophageal stenosis between the four groups (P = 0.887,0.711); there was no significant difference in the short term efficacy (P = 0.862); acute reactive radiation tracheitis, radioactive esophagus There was significant difference between the two groups (P = 0.049, 0.031). The two groups were significantly higher than the conventional group. The local control rate and survival rate were significantly different between the two groups (P = 0.028,0.036,0.040,0.014). [Conclusion] Hyperfraction radiotherapy aggravates acute response and DNA polβ expression has different effect on radiosensitivity. The prognosis of hyperfraction radiotherapy in low expression group is relatively good.
