目的探讨三磷酸肌醇受体(IP3R)在妊娠期高血压病(GHD)产妇胎盘组织的表达及临床意义。方法行剖宫产分娩的GHD产妇300例分为轻度子痫前期(A组,218例)和重度子痫前期(B组,82例);同期住院行剖宫产分娩的正常妊娠产妇200例作为对照(C)组。以免疫组化SP法检测产妇胎盘组织的IP3R表达。结果在正常妊娠和子痫前期患者胎盘组织中均有不同程度的IP3R表达,以滋养细胞及蜕膜组织中分布较多,绒毛血管内皮细胞及部分间质细胞液偶见其表达。A组和B组产妇胎盘绒毛滋养细胞IP3R表达强度均高于C组(强阳性检出率:44.5%和67.1%vs.15.0%),B组高于A组(P<0.05)。A组和B组产妇蜕膜组织中IP3R表达强度均高于C组(强阳性检出率:42.7%和62.3%vs.13.5%),B组高于A组(P<0.05)。结论在GHD产妇胎盘组织中,IP3R表达上调,提示其可能参与了GHD的发生及发展过程。
Objective To investigate the expression and clinical significance of inositol 1,4,5 - trisphosphate receptor (IP3R) in placenta of pregnant women with hypertensive disorder of pregnancy (GHD). Methods Three hundred and seventy pregnant women with GHD delivered by cesarean section were divided into mild preeclampsia (group A, n = 218) and severe preeclampsia (group B, n = 82). The normal pregnant women with cesarean delivery during the same period Example as a control (C) group. Immunohistochemical SP method was used to detect the expression of IP3R in placenta of pregnant women. Results There were different degrees of IP3R expression in normal pregnancy and preeclampsia placenta tissues, with more distribution in trophoblast and decidua, and villous vascular endothelial cells and some interstitial fluid occasionally seen. The expression of IP3R in placental trophoblasts in group A and group B was higher than that in group C (strong positive detection rate: 44.5% and 67.1% vs.15.0%, respectively), but higher in group B than in group A (P <0.05). The expression intensity of IP3R in decidua tissues of group A and group B were higher than that of group C (strong positive detection rate: 42.7% and 62.3% vs.13.5%, respectively), and group B was higher than that of group A (P <0.05). Conclusion Upregulation of IP3R expression in GHD maternal placenta suggests that it may be involved in the occurrence and development of GHD.
