目的探讨阴道和宫颈尖锐湿疣(CA)组织中人乳头瘤病毒(HPV)感染基因型分布情况及其临床意义。方法采用PCR和基因芯片技术对56例阴道CA组织和62例宫颈CA组织标本进行23种HPV基因分型检测,并对其患者进行临床病理资料分析。结果 56例阴道CA组织中检出HPV阳性感染51例,感染率为91.07%;其中一重HPV感染32例,感染率为57.14%;多重HPV感染19例,感染率为33.93%;一重感染中HPV11型19例,HPV6型11例;多重HPV感染中HPV6+11型4例,HPV11+52型3例。62例宫颈CA组织中检出HPV阳性感染59例,感染率为95.16%;其中一重HPV感染45例,感染率为72.58%;多重HPV感染14例,感染率为22.58%;一重感染中HPV11型29例,HPV6型14例;多重HPV感染中HPV6+11型4例,HPV11+18型2例。结论 HPV6、11、6+11、11+52型是阴道CA的主要致病类型,HPV6、11、6+11、11+18型是宫颈CA的主要致病类型。PCR结合基因芯片技术是一种比较适合临床进行HPV分型诊断的方法,其敏感性高,特异性好,对我国女性阴道和宫颈CA及其癌瘤的防治以及疫苗的研究具有十分重要的意义。
Objective To investigate the genotype distribution of human papillomavirus (HPV) infection in vaginal and cervical condyloma acuminatum (CA) and its clinical significance. Methods Twenty-three HPV genotypes were detected in 56 cases of vaginal CA tissue and 62 cases of cervical CA tissue samples by PCR and gene chip technology, and their clinical and pathological data were analyzed. Results Fifty-five cases of HPV positive infection were detected in 56 cases of vaginal CA, the infection rate was 91.07%. Among them, 32 cases were infected with one type of HPV, the infection rate was 57.14%, 19 cases with multiple HPV infection, the infection rate was 33.93% 19 cases of HPV type, 11 cases of HPV type 6, 4 cases of HPV type 6 + 11 and 3 cases of HPV type 11 + 52 in multiple HPV infection. In 62 cases of cervical CA tissue, 59 cases were positive for HPV infection, the infection rate was 95.16%. Among them, 45 cases were infected with one type of HPV, the infection rate was 72.58%, 14 cases with multiple HPV infection, the infection rate was 22.58% 29 cases, HPV6 14 cases; multiple HPV infection in HPV6 + 11 in 4 cases, HPV11 +18 in 2 cases. Conclusion HPV6, 11, 6 +11 and 11 +52 are the main causative agents of vaginal CA. HPV6, 11, 6 +11 and 11 +18 are the major causative agents of cervical CA. PCR combined with gene chip technology is a more suitable method for clinical diagnosis of HPV typing, its high sensitivity and specificity of the female vaginal and cervical CA and its prevention and treatment of cancer and vaccine research is of great significance .
