目的:探讨对社区2型糖尿病病人开展签约社区家庭医生模式护理干预的影响。方法:将120例2型糖尿病病人分对照组和签约组,并进行为期1年的健康管理。签约组由医生、护士、预防保健医师组成团队,依照糖尿病病人的实际情况制订对应的护理干预措施(包括构建健康档案、并发症护理、运动护理、心理护理、饮食护理以及健康教育等),同时实施追踪护理,比较2组管理后糖尿病相关知识、遵医行为、空腹血糖、餐后2小时血糖、糖化血红蛋白、体质指数变化。结果:实施健康管理1年后,签约组糖尿病病人的糖尿病相关知识、遵医行为、空腹血糖、餐后2小时血糖、糖化血红蛋白、体质指数均比对照组明显降低,其差异有统计学意义(P<0.05)。结论:签约社区家庭医生模式健康管理对于社区2型糖尿病病人的综合控制有明显效果。
Objective: To explore the impact of nursing intervention on community-based family doctors in community type 2 diabetic patients. Methods: 120 patients with type 2 diabetes were divided into control group and contracted group, and a one-year health management. The contracting team consists of teams of doctors, nurses and preventive medicine practitioners to formulate corresponding nursing interventions (including building health records, complication care, sports nursing, psychological nursing, diet nursing and health education) according to the actual situation of patients with diabetes mellitus Implementation of follow-up care, diabetes management-related knowledge, compliance, fasting blood glucose, 2-hour postprandial blood glucose, glycosylated hemoglobin, body mass index were compared between the two groups. Results: One year after the implementation of health management, the diabetes-related knowledge, compliance behavior, fasting blood glucose, postprandial 2-hour postprandial blood glucose, glycosylated hemoglobin and body mass index of contracted diabetes patients were significantly lower than those of the control group (P < P <0.05). CONCLUSIONS: The health management of family doctor in contracted community has obvious effect on the comprehensive control of community type 2 diabetes patients.
