药敏试验是临床选择敏感抗生素的精确检测工作,其成败在于质量控制的可靠性和最低抑菌浓度(MIC)的精确性、前瞻性.本文结合质控试验和药敏试验对梯度平板法进行验证,梯度平板法所测得的MIC与微量肉汤稀释法和琼脂稀释法测得的MIC结果一致,且因为线性设置的药物浓度,MIC更加精确,能推算抗菌药物的原始浓度,更好地保证质控试验的可靠性.同时,按照美国临床实验室标准委员会或欧盟药敏试验委员会的敏感(S),中介(I)以及抗药(R)范围标准设置0-S,S-R,R-8R这样3个线性梯度,首先判断菌株敏感性范围,然后精确测定细菌群落的MIC值,并能有效分离出抗性菌株,预警该抗生素使用后可能出现的抗药性,进而指导临床抗菌药物的选择和应用.梯度平板的主要困难在于手工制备的繁琐和不稳定性,如果自动化生产即用型梯度平板,将便利临床药敏试验,改进抗药性筛选技术.
