目的:探讨低分子肝素钠预防患儿过敏性紫癜肾炎的临床机制。方法:将60例患儿随机分为治疗组(30例)和对照组(30例)。治疗组在常规治疗基础上给予低分子肝素钠。观察两组临床症状消失时间及尿常规异常情况。比较两组疗效、肾损害的情况及比较治疗前、治疗1个月、3个月后复查尿微量白蛋白(Alb)、β2-微球蛋白(β2-mG)含量的变化以及血清IL-8的水平。结果:治疗组临床症状消失时间,尿常规异常明显低于对照组。两组疗效比较,差异无统计学意义(P>0.05)。治疗组肾损害发生率低于对照组。治疗1个月后、3个月后治疗组尿Alb及β2-mG和血清IL-8水平均明显低于对照组(P<0.05)。结论:低分子肝素钠能预防或减少HSPN的发生,具有显著的肾脏保护作用,可能与其降低血清IL-8水平有关。
Objective: To investigate the clinical mechanism of low molecular weight heparin in preventing allergic purpura nephritis in children. Methods: Sixty children were randomly divided into treatment group (30 cases) and control group (30 cases). The treatment group was given low molecular weight heparin sodium on the basis of routine treatment. The disappearance of clinical symptoms and urinary abnormalities were observed in the two groups. The changes of therapeutic effect and renal damage were compared between the two groups before treatment, 1 month after treatment and 3 months later. The changes of urinary albumin (Alb), β2-microglobulin (β2-mG) and serum IL-8 s level. Results: The disappearance of clinical symptoms and urinary abnormalities in the treatment group were significantly lower than those in the control group. There was no significant difference between the two groups (P> 0.05). The incidence of renal damage in the treatment group was lower than that in the control group. After 1 month of treatment, urinary Alb, β2-mG and IL-8 levels in the treatment group were significantly lower than those in the control group after 3 months (P <0.05). CONCLUSION: Low molecular weight heparin can prevent or reduce the occurrence of HSPN and has significant renal protective effect, which may be related to the decrease of serum IL-8 level.
