目的 :分析儿童癫发作的临床表现和录像脑电图 (Video -EEG)特征。方法 :对 12 3例年龄 9天至 15岁癫患儿进行Video -EEG监测 ,记录并同步分析发作时EEG与临床表现。结果 :12 3例中记录到 5 6 8次发作 ,分三种类型。 1、全面性发作 5 3例 2 0 1次发作 :①婴儿痉挛 18例中 5 3次发作 ,发作期EEG表现为广泛低波幅快活动或高波幅慢波暴发与低电压抑制 ;②肌阵挛发作 12例中 79次发作 ,发作期EEG表现各导联多棘波、多棘慢波 ;③强直发作 10例中 2 4次发作 ,发作期EEG表现导联低波幅快节律、多棘慢波 ;④阵挛发作 10例中 39次发作 ,发作期EEG表现各导联棘慢波、尖慢波 ;⑤失神发作 3例中 6次发作 ,发作期EEG表现全导联 3Hz棘慢波。 2、部分性发作 6 7例中 339次发作 ,分别起源于额、中央、顶、颞、枕 ,其中 38例 191次发作为继发全身发作。 3、混合发作 3例中 2 8次发作。结论 :Video -EEG有助于临床医师对儿童癫的正确分型及指导治疗。
Objective: To analyze the clinical manifestations of children with epileptic seizures and video-EEG features. Methods: Video-EEG was performed on 123 children with epilepsy aged 9 to 15 years old. EEG and clinical manifestations were recorded and analyzed synchronously. Results: Fifty-eight seizures were recorded in 123 cases, divided into three types. 1, comprehensive seizures 53 cases 20 cases of seizures: ① infantile spasms in 18 cases of 5 3 seizures, episodes of EEG showed extensive low-amplitude fast activity or high-amplitude slow-wave outbreaks and low voltage suppression; ② myoclonus Seizures in 12 cases, 79 episodes, the onset of EEG performance of each lead spike, spike and spike; ③ 10 cases of tonic seizures in 24 episodes, the onset of EEG performance guide low amplitude fast rhythm, spike spike ; ④ clonic seizures in 10 cases, 39 episodes, the onset of EEG performance of each lead spines slow wave, sharp slow wave; ⑤ loss of seizures in 3 cases in 6 seizures, epileptic EEG showed all-lead 3Hz spikes and waves. 2, partial seizures 67 cases of 339 episodes, respectively, originated in the frontal, central, top, temporal, occipital, of which 191 cases of 38 cases of seizures secondary to systemic seizures. 3, mixed attack in 3 cases, 28 episodes. Conclusion: Video-EEG helps clinicians correct the classification of children’s epilepsy and guide the treatment.
