[目的]探讨贝那普利、缬沙坦联合治疗老年糖尿病肾病(DN)的疗效及对患者血糖、血脂、肾功能的影响.[方法]98例老年DN患者随机分为两组.对照组在常规治疗基础上加用贝那普利,观察组在常规治疗基础上加用贝那普利和缬沙坦.比较治疗前、连续治疗6个月后患者血糖、血脂、肾功能等指标变化情况及药物不良反应发生率.[结果]两组治疗前各观察指标比较差异均无显著性(P>0.05);治疗6个月后两组空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、甘酰三油(TG)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、尿白蛋白排泄率(UAER)、24h尿蛋白定量(24h Upro)、血肌酐(SCr)、血尿素氮(BUN)较治疗前均显著下降,而HDL-C则显著上升(均P<0.05);且观察组血脂、肾功能指标改善效果优于对照组(P<0.05);二组不良反应发生情况比较差异无显著性(P>0.05).[结论]贝那普利、缬沙坦联合治疗老年DN患者能提高临床疗效,获得更为理想的血糖、血脂、肾功能改善效果.“,”[Objective]To investigate the therapeutic effects of benazepril combined with valsartan on blood glucose,blood lipid and kidney function in treatment of senile diabetic nephropathy(DN).[Methods]A total of 98 patients with senile DN were randomly split into two groups.Patients in the control group were given benazepril on the basis of routine treatment and patients in the study group were given both benazepril and valsartan on the basis of routine treatment.The changes of blood glucose,blood lipid,renal function and adverse drug reactions were compared between the two groups before and after 6-months of treatment.[Results]Before the treatment,the above parameters in the two groups were not significantly different (P >0.05).After 6 months of treatment,fasting blood glucose (FPG),postprandial 2h blood glucose (2hPG),glycated hemoglobin (HbA1c),glycerol triolein (TG),total cholesterol (TC),lowdensity lipoprotein cholesterol (HDL-C),urine albumin excretion rate (UAER),24h urine protein quantitation (24hUpro),serum creatinine (SCr),and urea nitrogen (BUN) in the two groups decreased significantly while high-density lipoprotein cholesterol (HDLC) level increased (P <0.05).The improvement in blood lipid levels and renal function in the study group were better than those in the control group (P <0.05).There were no significant differences in the adverse reactions between the two groups (P >0.05).[Conclusion]Benazepril combined with valsartan can lead to heightened therapeutic effects and better blood glucose levels,blood lipid levels and kidney function in the treatment of senile diabetic nephropathy.
