目的探讨氧气驱动雾化吸入用复方异丙托溴铵溶液治疗支气管哮喘急性发作期的临床治疗效果。方法 50例支气管哮喘急性发作期患者,随机分为治疗组(26例)和对照组(24例)。对照组患者采用解痉、平喘、抗炎等对症治疗,治疗组患者在对照组基础上给予氧气驱动雾化吸入用复方异丙托溴铵溶液治疗,比较两组患者治疗前及治疗3 d后桡动脉血中血氧分压(PaO_2)、二氧化碳分压(PaCO_2)指标以及静脉血中白三烯B4(LTB4)的含量。结果治疗组总有效率为96%,明显高于对照组的67%,差异有统计学意义(P<0.05)。治疗后,两组患者PaCO_2平均值均低于治疗前,PaO_2平均值均高于治疗前,且治疗组PaCO_2、PaO_2平均值均优于对照组,差异有统计学意义(P<0.05)。治疗后,两组LTB4含量均低于治疗前,且治疗组低于对照组,差异有统计学意义(P<0.05)。治疗组有3例出现腹痛、腹泻、恶心、呕吐等症状,不良反应发生率为11.5%(3/26),对照组也有3例出现恶心呕吐、腹痛、腹泻等症状,不良反应发生率为12.5%(3/24);治疗组不良反应发生率与对照组比较差异无统计学意义(P>0.05)。结论氧气驱动雾化吸入用复方异丙托溴铵溶液用于支气管哮喘急性发作期的治疗,不仅显著缓解了患者的症状,而且提高了患者的康复、生存质量,值得广大临床医生借鉴、应用、推广。
Objective To investigate the clinical effect of aerosol inhalation with compound ipratropium bromide in the treatment of acute exacerbation of bronchial asthma. Methods Fifty patients with acute exacerbation of bronchial asthma were randomly divided into treatment group (n = 26) and control group (n = 24). Patients in the control group were treated with antispasmodic, antiasthmatic, anti-inflammatory and other symptomatic treatment. The patients in the treatment group were treated with oxygen-driven inhalation of compound ipratropium bromide on the basis of the control group. Before and 3 days of treatment, (PaO_2), PaCO_2 index and venous blood leukotriene B4 (LTB4) content in posterior radial artery. Results The total effective rate was 96% in the treatment group, which was significantly higher than 67% in the control group (P <0.05). After treatment, the average PaCO_2 in both groups was lower than that before treatment, and the average value of PaO_2 was higher than that before treatment. PaCO_2 and PaO_2 in the treatment group were better than those in the control group (P <0.05). After treatment, the levels of LTB4 in both groups were lower than before treatment, and the treatment group was lower than the control group, the difference was statistically significant (P <0.05). In the treatment group, there were 3 cases of abdominal pain, diarrhea, nausea, vomiting and other symptoms, the incidence of adverse reactions was 11.5% (3/26), nausea and vomiting, abdominal pain, diarrhea and other symptoms in the control group, the incidence of adverse reactions was 12.5 % (3/24). There was no significant difference in the incidence of adverse reactions between the treatment group and the control group (P> 0.05). Conclusion Oxygen-driven atomization inhalation with compound ipratropium bromide solution for the treatment of acute exacerbation of bronchial asthma, not only significantly alleviate the symptoms of patients, but also improve the patient’s rehabilitation, quality of life, it is worth drawing the majority of clinicians, applications, Promotion.
