目的探讨血清脂联素水平与胰岛素抵抗之间的关系,以及肥胖和胰岛素抵抗的交互作用对脂联素的影响。方法将167例研究对象分成胰岛素抵抗伴肥胖、单纯胰岛素抵抗、单纯肥胖和正常状态四组,运用方差分析、协方差分析对观察指标进行研究。结果在扣除性别、收缩压、舒张压、甘油三酯和高密度脂蛋白胆固醇对血清脂联素水平的线性影响后,脂联素在四组中的修正均数依次为3.477μg/mL、3.681μg/mL、4.447μg/mL和4.614μg/mL(P=0.033),胰岛素抵抗伴肥胖组的血清脂联素水平明显低于单纯肥胖组(P=0.036)和正常组(P=0.009),未发现胰岛素抵抗和肥胖的交互作用的存在。结论血清脂联素水平降低与胰岛素抵抗之间互为因果,肥胖和胰岛素抵抗均与脂联素水平下降独立相关。
Objective To investigate the relationship between serum adiponectin level and insulin resistance and the effect of obesity and insulin resistance on adiponectin. Methods 167 subjects were divided into four groups: insulin resistance with obesity, simple insulin resistance, simple obesity and normal state. The variance and covariance analysis were used to study the observation. Results After adjusting for the linear effects of gender, systolic blood pressure, diastolic blood pressure, triglyceride and high-density lipoprotein cholesterol on serum adiponectin levels, the corrected mean numbers of adiponectin in the four groups were 3.477 μg / mL, 3.681 The level of serum adiponectin in obesity-resistant group was significantly lower than that in simple obesity group (P = 0.036) and normal group (P = 0.009) No interaction between insulin resistance and obesity was found. Conclusions The decrease of serum adiponectin and insulin resistance are mutually causal, and obesity and insulin resistance are independently related to the decrease of adiponectin level.
