目的检测甲状腺乳头状增生、结节性甲状腺肿和甲状腺乳头状癌组织中淋巴管特异标记物D2-40的表达和微淋巴管密度(MLVD),探讨其临床意义及相互关系。方法应用单克隆抗体D2-40检测15例甲状腺乳头状增生病变、10例结节性甲状腺肿和53例甲状腺乳头状癌周围组织中的D2-40的表达,计算其微淋巴管密度(MLVD),分析其与淋巴结转移之间的关系。结果D2-40选择性表达于甲状腺的淋巴管内皮细胞。淋巴结转移阳性组淋巴管密度明显高于淋巴结阴性组、甲状腺乳头状增生组和结节性甲状腺肿组,差异有统计学意义。结论检测D2-40表达能敏感和特异地检测出肿瘤的MLVD,其对预测淋巴结微转移具有可靠性。可望有良好的临床应用价值。
Objective To detect the expression of lymphatic-specific marker D2-40 and the lymphatic vessel density (MLVD) in papillary thyroid hyperplasia, nodular goiter and papillary thyroid carcinoma and to explore its clinical significance and its relationship. Methods The expression of D2-40 in 15 cases of thyroid papillary hyperplasia, 10 cases of nodular goiter and 53 cases of thyroid papillary carcinoma was detected by monoclonal antibody D2-40, and the lymphatic vessel density (MLVD) , Analyze its relationship with lymph node metastasis. Results D2-40 selectively expressed in thyroid lymphatic endothelial cells. Lymphatic vessel density positive lymph node metastasis was significantly higher than the negative lymph node group, papillary thyroid hyperplasia group and nodular goiter group, the difference was statistically significant. Conclusion Detecting D2-40 expression can detect MLVD of tumor sensitively and specifically, which is reliable for predicting micrometastasis of lymph nodes. Expected to have a good clinical value.
