目的探讨免疫抑制剂治疗系统性红斑狼疮(SLE)患者淋巴细胞亚群失调的疗效。方法选取2014年1月至2015年7月佛山市第一人民医院收治的60例SLE患者为研究对象,其中未治疗和经免疫抑制剂治疗患者各30例,同时选取同期健康体检者60名作为对照组。采用流式细胞仪检测SLE组和对照组患者外周血T淋巴细胞、B淋巴细胞和自然杀伤细胞(NK)的比例,采用ELISA检测血清干扰素-γ(IFN-γ)及白细胞介素-4(IL-4)表达水平。结果 SLE组患者CD3+CD4+Th细胞比例、Th细胞/Ts细胞、CD3-CD16+CD56+NK细胞比例均明显低于对照组CD3+CD8+Ts细胞、CD3-CD19+B淋巴细胞比例均明显高于对照组,差异均有统计学意义(均P<0.05);与对照组相比,SLE组患者血清IFN-γ水平、IFN-γ/IL-4比值均显著增高,而IL-4水平明显降低,差异均有统计学意义(均P<0.05);使用免疫抑制剂治疗后,患者CD3+CD4+T淋巴细胞比例明显增加,CD3-CD19+B淋巴细胞比例明显降低,Th细胞/Ts细胞比值明显高于未治疗组,差异均有统计学意义(均P<0.05)。结论 SLE患者经常用免疫抑制剂治疗后,Th细胞/Ts细胞比例得到有效调节和改变,B细胞数量下降,而且外周血淋巴细胞各亚群比例向正常方向调整,有效改善了患者淋巴细胞亚群失调状况。
Objective To investigate the effect of immunosuppressive agents on the imbalance of lymphocyte subsets in patients with systemic lupus erythematosus (SLE). Methods Sixty patients with SLE admitted to Foshan First People’s Hospital from January 2014 to July 2015 were enrolled in this study. 30 untreated patients and 30 immunosuppressed patients were enrolled in this study. Control group. The ratio of T lymphocytes, B lymphocytes and natural killer cells (NK) in peripheral blood of SLE group and control group was detected by flow cytometry. The levels of serum IFN-γ and IL-4 (IL-4) expression level. Results The proportion of CD3 + CD4 + Th cells, Th cells / Ts cells and CD3-CD16 + CD56 + NK cells in SLE group were significantly lower than those in control group, and the proportion of CD3-CD19 + B lymphocytes was significantly (P <0.05). Compared with the control group, the levels of IFN-γ and IFN-γ / IL-4 in SLE group were significantly higher than those in control group, while the levels of IL-4 (P <0.05). After treatment with immunosuppressive agents, the proportion of CD3 + CD4 + T lymphocytes was significantly increased, while the proportion of CD3-CD19 + B lymphocytes was significantly decreased. Th cells / Ts Cell ratio was significantly higher than untreated group, the difference was statistically significant (P <0.05). CONCLUSIONS: Th cells / Ts ratio is effectively regulated and changed in SLE patients after immunosuppressant treatment. The number of B cells is decreased, and the proportion of peripheral blood lymphocyte subsets is adjusted in the normal direction, which effectively improves the lymphocyte subsets Disorders.
