目的比较胰岛素混合液与康惠尔渗液对Ⅱ期以上压疮患者肉芽组织生长及创面血管化的影响。方法选取本院2014年2月至2016年2月100例被确诊患有Ⅱ期以上压疮的患者,按统计学随机数字表将患者随机分为胰岛素混合液组(52例)、康惠尔渗液组(48例),以创面愈合时间的长短等作为评价指标,应用统计学方法对实验结果进行分析。结果胰岛素混合液组、康惠尔渗液组Ⅲ期患者创面愈合时间比较差异无统计学意义(t=1.724,P>0.05)。胰岛素混合液组、康惠尔渗液组Ⅳ期患者创面愈合时间比较差异无统计学意义(t=1.939,P>0.05)。患者治疗两周后Ⅲ期压疮创面胰岛素混合液组治疗有效率为65.2%,康惠尔渗液组治疗有效率为68.4%,两组治疗有效率差异无统计学意义(χ~2=1.265,P<005);Ⅳ期压疮创面胰岛素混合液组治疗有效率为33.3%,康惠尔渗液组治疗有效率为40.0%,两组差异有统计学意义(χ~2=5.658,P<0.05)。各阶段胰岛素混合液组、康惠尔渗液组创面面积减小值、创面肉芽面积变化值比较差异无统计学意义(P均>0.05)。结论胰岛素混合液治疗Ⅱ期以上压疮患者效果显著,和现代新型敷料康惠尔渗液的治疗效果相近;但是胰岛素的使用成本相对于康惠尔渗液成本低廉,易被广大患者接受。
Objective To compare the effects of insulin and Kang Wyle exudate on granulation tissue growth and wound vascularization in patients with pressure ulcer over stage Ⅱ. Methods From February 2014 to February 2016, 100 patients who were diagnosed as having stage Ⅱ or more pressure ulcers from February 2014 to February 2016 were randomly divided into insulin mixture group (52 cases) according to statistical random number table, Exudate group (48 cases), wound healing time as the evaluation index, the application of statistical methods to analyze the experimental results. Results There was no significant difference in the wound healing time between the insulin mixture group and Kangwureier exudate group (t = 1.724, P> 0.05). There was no significant difference in wound healing time between insulin mixture group and Kangwureier exudate group (Ⅳ) (t = 1.939, P> 0.05). After two weeks of treatment, the effective rate of treatment of insulin mixture in stage Ⅲ pressure ulcer wound was 65.2% and that of Kangwell exudate group was 68.4%. There was no significant difference in the treatment efficiency between the two groups (χ ~ 2 = 1.265 , P <005). The effective rate of treatment in stage Ⅳ pressure ulcer wound insulin was 33.3% and that in Kangwell exudate group was 40.0%, the difference was statistically significant (χ ~ 2 = 5.658, P <0.05). There were no significant differences in the change of the wound area between the insulin mixture group and KangWheer exudate group at each stage (P all> 0.05). Conclusions Insulin mixture is effective in the treatment of patients with pressure ulcer over stage Ⅱ, which is similar to that of Kangweier exudate of modern new dressing. However, the cost of using insulin is relatively lower than that of Kangwill exudation and is easily accepted by the vast majority of patients.
