1 病案存在的主要问题 1.1 病案首页:如疾病名、手术名不规范,损伤原因填写不具体等。 1.2 主诉:常以诊断代替主诉,主诉叙述太长,主次颠倒。 1.3 病史:重点不突出、主诉和现病史脱节,重要鉴别资料不具体。 1.4 诊断方面:诊断依据不充分,诊断不及时,诊断未按ICD-9书写、主次诊断排序不科学等。 1.5 治疗方面:治疗不及时,治疗不合理,治疗不全面、不彻底。
