目的通过总结高海拔地区下肢深静脉血栓患者的彩色多普勒超声(彩超)表现(CDFI),以期达到临床上尽早诊断,及时治疗,减少并发症的发生。方法 76例高海拔地区下肢深静脉血栓患者,对比分析患者双侧下肢深静脉的彩超表现,并分析其基础病因。结果 76例高海拔地区下肢深静脉血栓患者中,28例急性期患者彩超表现为低回声团充填,43例亚急性期患者表现为稍强回声团充填,5例慢性期患者表现为强回声团充填,静脉管壁毛糙增厚明显,管腔内径正常或变小;均为单侧发病,左下肢41例,右下肢35例;26例发生在股静脉,18例发生在腘静脉,32例发生在股静脉-腘静脉-胫前后静脉;基础疾病中,肿瘤14例,糖尿病14例,心功能不全8例,高脂血症16例,贫血12例,外伤8例,病因不明4例。结论高海拔地区下肢深静脉血栓患者的彩超表现典型,特别是伴有基础疾病的患者,应该常规做下肢深静脉彩超检查,以尽早发现深静脉血栓,减少并发症的发生。
Objective To summarize the color Doppler ultrasonography (CDFI) in patients with deep venous thrombosis of lower extremity in high altitude area, in order to achieve the clinical diagnosis as soon as possible and timely treatment to reduce the incidence of complications. Methods 76 patients with deep venous thrombosis of lower extremity in high altitude region were studied. The color Doppler echocardiography of bilateral lower extremity deep vein was comparatively analyzed and the underlying etiology was analyzed. Results Among the 76 patients with deep venous thrombosis in the high altitude region, the ultrasonography was performed in 28 cases of acute phase, the hypoechoic group was filled in 43 cases of subacute phase, the slightly hyperechoic group was filled in 43 cases of subacute phase, and the hyperechoic group was found in 5 cases of chronic phase Filling, thick wall thickening of the vein obvious, lumen diameter normal or smaller; both unilateral disease, left lower limb in 41 cases, right lower limb in 35 cases; 26 cases occurred in the femoral vein, 18 cases occurred in the popliteal vein, 32 cases Occurred in the femoral vein - 腘 vein - anterior tibial vein vein; 14 cases of underlying diseases, diabetes, 14 cases, 8 cases of heart failure, hyperlipidemia in 16 cases, anemia in 12 cases, trauma in 8 cases, 4 cases of unknown etiology. Conclusion The color Doppler echocardiography of patients with deep venous thrombosis of lower extremities in high altitude is typical. Especially in patients with underlying diseases, deep venous ultrasonography should be routinely performed to detect deep vein thrombosis and reduce the incidence of complications.
