患者,女,75岁。以发作性右上腹疼痛10年,加重半年之主诉入院。查体:右上腹胆囊区明显压痛,腹部B超提示:结石性胆囊炎;肝左叶回声不均。行剖腹探查,胆囊切除术。术中见肝左叶有多个结节突出于表面,直径1～1.5cm,外观樱桃色,中心发白,略凹陷(切除送病检)。胆囊内充有多面体结石7枚,直径0.8cm左右,胆囊粘膜面光滑,质软。探查胆管、胃肠道均未见异常,腹腔内亦未见肿大淋巴结。整个手术顺利,术后患者恢复良好,2周后痊愈出院。
Patient, female, 75 years old. With a paroxysmal right upper quadrant pain for 10 years, the chief complaint was added to the hospital for six months. Physical examination: marked tenderness in the gallbladder area of the right upper abdomen, abdominal B-mode ultrasound: cholecystitis; left liver lobe echo uneven. Laparotomy, cholecystectomy. During the operation, the left hepatic lobe had multiple nodules protruding from the surface, with a diameter of 1 to 1.5 cm, a cherry appearance, a white center, and a slight depression (resection of the disease). The gallbladder is filled with seven polyhedral stones, with a diameter of 0.8 cm, and the gallbladder mucosa is smooth and soft. No abnormalities were detected in the bile ducts and gastrointestinal tract, and no enlarged lymph nodes were found in the abdominal cavity. The entire operation was successful and the patient recovered well after surgery. He was discharged after 2 weeks.
