现行公费医疗管理体制是一种大锅饭的体制,财政拿钱,病人拿药,医疗单位、享受公费医疗人员及其工作单位均不负任何经济责任。特别是有些医疗单位,片面追求经济收入,放松公费医疗管理,大量购进、推销非治疗性药品;有些医生开大处方、人情方,随病人指名要药,造成公费医疗大量超支。我区自1980年至1984年,五年来共超支6,533万元,平均每年递增50%。要根本扭转公费医疗开支大幅度超支的局面,必须改变现行的公费医疗管理形式,使公费医疗管理与有关医疗单位和个人经济利益挂钩,明确公费医疗管理的经济责任。
The current public-funded medical management system is a system of large pots. The financial personnel take money, patients take drugs, medical units, public-funded medical personnel and their work units do not bear any economic responsibility. In particular, some medical units unilaterally pursue economic income, relax publicly funded medical management, and purchase and sell non-therapeutic drugs in large quantities; some doctors prescribe large prescriptions, human emotions, and require medicines as the patients are named, resulting in large overspending of publicly funded medical services. From 1980 to 1984 in our district, the total expenditure over the five years was 65.33 million yuan, an average annual increase of 50%. To fundamentally reverse the situation in which public expenditure on medical expenses is drastically exceeded, we must change the current form of publicly funded medical management, link the management of publicly funded medical care with the economic interests of relevant medical units and individuals, and clarify the economic responsibilities of publicly funded medical management.
